Abstract

INTRODUCCIÓN: La demencia es un síndrome que se caracteriza por dotar a una persona de un deterioro adquirido de la función cognitiva y causa un impacto importante a nivel físico, psicológico, social y económico tanto para el propio sujeto cómo para los familiares o cuidadores que se encargan de él. A nivel global hay unos 50 millones de personas con demencia y, cada año, se registran unos 10 millones de casos nuevos. Contar con un proceso que ayude a estos pacientes y familiares durante la enfermedad favorece su calidad de vida; este es, el proceso de voluntades anticipadas. OBJETIVO: El objetivo de este proyecto es describir los beneficios de la realización de un plan de voluntades anticipadas con el fin de que aumente la prevalencia de estos en pacientes con demencias y, se disminuya el sufrimiento y el dolor durante la enfermedad y, en el proceso de final de vida mediante la implicación del colectivo de enfermería de Atención Primaria y valorando la opinión de los familiares del paciente con demencia. MÉTODO: Se propone la realización de un seguimiento mediante encuestas de los equipos de atención primaria que realicen Procesos de Voluntades Anticipadas del Centro de Urgencias de Atención Primaria Horta y del Centro de Atención Primaria Gracia para valorar su percepción en relación a las mejoras de los pacientes con demencia que inician un PVA en etapas iniciales de la enfermedad. También se propone conocer la percepción de los familiares a nivel de control del dolor y del sufrimiento durante la enfermedad y en SUD para evaluarla y poder realizar cambios o mejoras en el seguimiento asistencial. CONCLUSIONES: las demencias cada vez presentan una mayor prevalencia en nuestra sociedad. Favorecer la difusión de la importancia de realizar un buen proceso de voluntades anticipadas entre los profesionales de enfermería y también, dar a conocer la importancia de realizar procesos de voluntades anticipadas entre los familiares de estos pacientes y en la sociedad en general, es algo que permitirá conocer de forma anticipada el pronóstico y evolución de la enfermedad, llevar un buen control del dolor y el sufrimiento del paciente, favorecerá la gestión de emociones, resolverá dudas, y hará que el equipo sanitario realice un cuidado enfocado a paciente y familia durante todo el proceso. INTRODUCTION: The dementia is a syndrome that it’s characterized by providing a person of an acquired impairment of the cognitive functionality and causes an important impact at the physical level, psychological, social and economic both for the subject himself and for the relatives or caregivers who take care of him. Globally there are about 50 million people with dementia and every year are registered about 10 million new cases. Having a process that helps these patients and family during the disease favors their life quality and this is the process of anticipated wills. OBJECTIVE: The objective of this project is to describe the benefits of carrying out a plan of advance directives in order to increase the prevalence from these in patients with dementia and decreases suffering and pain during the illness and in the end-of-life process through the involvement of the nursing primary care community and valuing the relatives of the dementias patient opinion. METHOD: it is proposed the realization of a follow-up from the primary attention crew that execute process of anticipated wills from Horta’s CUAP and Gracia’s CAP to assess their perception in relation about the improvements in patients with dementia that they initiate a planning of anticipated wills in the initial stages of the disease. It’s also a proposal to know the perception of the family members at the level of pain control and suffering during the illness and in the end-of-life to evaluate and be able to make changes or improvements in the follow-up care. CONCLUSIONS: in our society dementia is increasingly a higher prevalence. Encourage dissemination of the importance of carrying out a good process of anticipated wills among the nursing professionals and also to make known the carrying out processes of advance directives it’s an important fact between the relatives of this patients and society in general and it’s something that will allow in advance the prognosis and evolution of the disease, leading to a good control of the patient’s pain and suffering, favoring the management of emotions and resolving doubts and will make the health team perform focused in patient and family care throughout the process.
