Abstract

Se presenta y discute el caso de una paciente de 45 años de edad, que sufrió resección intestinal masiva, quedando sólo con 80 centímetros de yeyuno, que se incorporó en anastomosis término-terminal a colon transverso. Se revisan los mecanismos de compensación fisiopatológica, los mismos que pueden durar más allá de los 2 años, siendo la opción conservadora, en opinión del autor, tolerable y menos riesgosa para la vida del paciente. Se revisa la necesidad de efectuar en estos pacientes colecistectomía profiláctica, soporte nutricional parenteral total, luego enteral, para finalmente ir añadiendo dietas más variadas pero pobres en grasa y lácteos. Debe proporcionarse otros refuerzos farmacológicos: Antagonistas H Loperamida, Gastrocinéticos y opcionalmente Somatostatina entre otros.

It's discuss a clinical report of a 45 years old patient, that suffered massive intestinal resection, remaining only 80 centimeters of yeyunum that was joined to the transverse colon in an end to end anastomosis. It' s review the physiopathologic compensation mechanisms that can last approximately two years, and in the author 's opinion the conservative option is tolerable, and less risky for the patient's life. It is pointed out the need in these patients to make a prophylactic colecystectomy, total parenteral nutrition support, then enteral, and finally adding more varied diets, poor in fats and milk products. It must be provided other drugs such as: H2 antagonists, loperamide, gastrokinetics and optionally somatostatine among others.
