Ketamina oral como medicación preanestésica pediátrica

Abstract

Buscamos en nuestro trabajo evaluar si la Ketamina oral es adecuada como medicación preanestésica atraumática en niños y establecer sus efectos colaterales luego de su administración oral. Cincuenta niños (Estado físico ASA I y II; entre 1 a 7 años de edad) fueron evaluados. Usando un método aleatorio a simple-ciego, prospectivamente asignados a los niños a uno de dos grupos (25 niños por grupo): grupo que no recibe Ketamina y grupo que se le administra 6 mg/kg de peso de Ketamina en 0.2 ml/kg de gaseosa. A todos se les evaluó el nivel de sedación y estado emocional, así como variables continuas durante la pre-inducción, anestesia y recuperación progresivamente en sus diversas etapas. Los valores de presión arterial, frecuencia cardíaca y respiratoria estuvieron dentro de límites normales, sin diferencia significativa entre los dos grupos. Hubo buena aceptación por parte de los niños de la Ketamina oral, presentándoseles sedación a los 20-25 minutos, una calmada separación de los padres y buenas condiciones para la inducción anestésica en un porcentaje elevado. Salvo secreciones incrementadas significativamente, no hubieron otros efectos indeseables colaterales. Se concluye que la Ketamina oral es adecuada como medicación pre-anestésica atraumática en niños, es de fácil administración, aceptada por la mayoría de los niños y económica; sin efectos colaterales indeseables significativos, a excepción de las secreciones incrementadas.

The purpose of our work was to evaluate if oral ketamine was adecuate as atraumatic pr e anesthetic medication in children, and to stablish it's side effects after the oral administration. Fifty children (physical status ASA I and II, between 1 and 7 years old) were evaluated. Using a simple blind random method, were prospectively assigned children to one of two groups (25 children per group) one group does not receive ketamine, and the other receives 6 mg/Kg of body weight of ketamine in 0.2 ml/Kg of beverage. Everybody was evaluated the level of sedation and emotional state, as well as continuous variables during the diverse fases of pre induction, anesthesia and recovery. The values of blood pressure, heart and respiratory rate were within normal limits, without any significant difference between the two groups. There was a good acceptance of the children to the oral ketamine, following sedation after 20-25 minutes, easy separation of the parents, and good conditions for the anesthetic induction in a high percentage. With the exception of significant increment of secretions, there were no other undesirable side effects. It is concluded that oral ketamine is adecuate as atraumatic pre anesthetic medication in children, it's easy to administer, accepted for most of the children and economic; without undesirable significant side effects, with the exception of incremented secretions.
