Utilidad del estudio inmunohistoquímico en el diagnóstico de lesiones premalignas de cérvix

Abstract

Diversos estudios muestran gran variabilidad interobservador en la interpretación microscópica de lesiones intraepiteliales (LIE) de cérvix, principalmente entre LIE de bajo grado (NIC I) y LIE de alto grado (NIC II y NIC III), así como también existe gran variabilidad en la interpretación lesiones pre malignas respecto de los procesos reactivos inflamatorios. El uso de p16 y ki67 ha demostrado mejorar la exactitud del diagnóstico para realizar dichas diferenciaciones, por lo cual podría ser aplicado como parte del protocolo de estudio de biopsias cervicales. En este estudio, se evalúa la confiabilidad diagnostica Intra e Inter-observador existente en el diagnóstico de muestras de cérvix evaluadas inicialmente con tinción histoquímica de H&E y posteriormente con tinción Inmunohistoquímica (IHQ) de KI67 y P16. Para este fin, primero se seleccionarán un total de 100 biopsias de cérvix que tengan diagnóstico previo de LIE de bajo o LIE de alto grado, y que además cuenten con estudio Inmunohistoquímico para p16 y ki67. A continuación, se realizará la evaluación histopatológica inicial por 2 patólogos expertos y se emitirá un primer informe histopatológico. Posteriormente, se evaluarán las láminas acompañadas de tinciones de Inmunohistoquímica y se emitirá un segundo informe histopatológico. Finalmente se evaluará la concordancia de ambos informes diagnósticos.
