Barreras para el uso del servicio dental con seguro de salud en una comunidad rural en el Perú

Abstract

Objetivo: Evaluar la asociación de las barreras de acceso al servicio dental entre los pobladores con tenencia de seguro de salud de una comunidad rural del Perú durante el 2018. Materiales y métodos: Estudio observacional, transversal. Basado en datos recolectados por las encuestas de salud oral realizada por el grupo de Waynanuna, del Departamento Académico de Odontología Social de la Universidad Peruana Cayetano Heredia en el 2018 en la población del Asentamiento Humano Fray Martin de Porras, Yaután, Ancash, Perú. Las variables analizadas fueron, dependiente: barreras de acceso al servicio dental, independiente: afiliación a un seguro de salud y co-variables: factores predisponentes individuales y factores facilitadores. Los datos obtenidos fueron analizados mediante el software estadístico Stata®. Se midió la diferencia entre la afiliación a un seguro de salud con las barreras de acceso al servicio dental, mediante la prueba de Chi-cuadrado, Prueba de Fisher y Razón de Prevalencia con un intervalo de confianza del 95%. Resultados: Se encontró una asociación negativa entre las barreras de acceso al servicio dental y las personas que cuentan con un seguro de salud (RP: 0.72, 95% IC 0.56-0.92). Además la asociación con la barreras fue: infraestructura (RP: 0.34, 95% IC 0.19-0.61), barrera de finanza (RP: 0.33, 95% CI 0.20-0.54) y barrera de necesidad (RP: 0.76, 95% IC 0.52-1.10) Conclusiones: Se encontró una asociación negativa entre las barreras de acceso al servicio dental y las personas que contaban con un seguro de salud.

Objective: To evaluate the association of barriers to access to dental services among residents with health insurance in a rural community in Peru during 2018. Methods: Observational, cross-sectional study. Based on data collected by oral health surveys conducted by Waynanuna's group, from the Academic Department of Social Dentistry of the Universidad Peruana Cayetano Heredia in 2018 in the population of the Fray Martin de Porras Human Settlement, Yaután, Ancash, Peru. The variables analyzed were dependent: barriers to access to dental services, independent: affiliation to health insurance and co-variables: individual predisposing factors and facilitating factors. The data obtained were analyzed using the Stata® statistical software. The difference between affiliation to health insurance and barriers to access to dental services was measured using the Chi-square test, Fisher's test and the prevalence ratio with a 95% confidence interval. Results: A negative association was found between barriers to access to dental services and people who have health insurance (PR: 0.72, 95% CI 0.56-0.92). In addition, the association with the barriers was: infrastructure (PR: 0.34, 95% CI 0.19-0.61), financial barrier (PR: 0.33, 95% CI 0.20-0.54) and necessity barrier (PR: 0.76, 95% CI 0.52 -1.10) Conclusion: A negative association was found between barriers to access to dental services and people who had health insurance.
