Sensibilidad antibiótica de bacterias aisladas en urocultivos positivos de un hospital general 2013-2017

Abstract

Antecedentes: El aumento de resistencia antibiótica de las bacterias productoras de infección del tracto urinario es un tema que repercute en el sistema de salud. El esquema de manejo varia y se acorta, motivo por el cual conocer la tasa de resistencia antibiótica ayuda a replantear y mejorar el tratamiento, por ello su estudio es importante en el Hospital Cayetano Heredia (HCH) para optimizar el tratamiento antibiótico. Objetivo: Describir la variación de la sensibilidad antibiótica de los principales agentes bacterianos causantes de infección del tracto urinario en un Hospital general en pacientes ambulatorios mayores de 18 años. Metodología: Estudio descriptivo retrospectivo, se incluyeron urocultivos positivos de pacientes ambulatorios y mayores de 18 años del HCH del periodo 2013 al 2017. Resultados: El uropatógeno más frecuente fue la E. coli con 78% de casos, seguido de las bacterias del genero Klebsiella con 8% y Pseudomonas aeruginosa con 2.4%. Se encontró un aumento de la frecuencia de bacterias productores de BLEE. E. coli fue sensible a amikacina y nitrofurantoina en más del 90%, Klebsiella a amikacina en más del 81% y Pseudomonas aeruginosa solo a colistina en el 100%. Conclusiones: La nitrofurantoina se administra por vía oral y la amikacina por vía intramuscular o endovenosa, han presentado menos del 20 % de resistencia requerida para ser un antibiótico de primera línea para la terapia empírica de las ITU en el HCH.
