Factores asociados a infección del tracto urinario adquirida en la comunidad por Escherichia coli productora de Beta-Lactamasas de espectro extendido

Abstract

En los últimos años se ha observado un aumento de las infecciones urinarias (ITU) causadas por bacterias productoras de beta-lactamasas de expectro extendido (BLEE). Sin embargo, es limitado el conocimiento acerca de la frecuencia y factores de riesgo en las ITU adquiridas en la comunidad. Estudios realizados en Europa y Asia identificaron diversos factores de riesgo, aunque en forma inconcluyente. Por este motivo se planteó el presente estudio retrospectivo analítico de casos y controles. Se revisaron los datos de los urocultivos provenientes de consultorio externo del Hospital Cayetano Heredia durante el año 2015 y se contactó por vía telefónica a los pacientes para la aplicación de un cuestionario basado en factores de riesgo identificados en estudios previos. Se contó con una población de 67 casos y 105 controles. Para el análisis estadístico se utilizó el paquete STATA versión 13. La frecuencia de ITU por E. coli productoras de BLEE fue de 40.85%. Según el análisis multivariado se identificó como factores de riesgo: uso de antibióticos (OR: 3,09), antecedente de hospitalización (OR: 2,92) y antecedente de cirugía (OR: 2,75). Estos factores fueron identificados también en estudios previos; sin embargo, en el presente estudio se identificó además el uso crónico de corticoides como un factor de riesgo (OR: 24,32; IC 95%: 2,39 – 246,92).
