Abstract

El personal militar desplegado en la Cuenca Amazonía está en alto riesgo de adquirir cutánea leishmaniasis (LC). Nosotros respondimos a un brote que ocurrido en el personal militar del Ejército peruano, que regresaron de corto entrenamiento en el Amazonas, se realizó la detección activa de casos, se recogió muestras de la lesión y se evaluó los factores de riesgo. La tasa de ataque fue del 25% (76/303); el período de incubación fue de 2-36 meses (media = 8). La mayoría de los casos tenían una lesión (66%), principalmente ulcerosa (49%), y en las piernas (57%). El tiempo real de reacción en cadena de la polimerasa (PCR) identificó Leishmania (V) braziliensis (59/61 = 97%) y L. (V.) guyanensis (2/61 = 3%). El sexo masculino (RR = 4,01; p = 0,034), no llevar ropa de manga larga (RR = 1,71; p = 0,005), y dormir en habitaciones abiertas (RR = 1,80; p = 0,009) se asociaron con LC. La terapia con Estibogluconato de sodio tuvo una tasa de curación del 41%, inferior al reportado previamente en Perú (~ 70%; p <0,001). Enfatizando la educación previa al despliegue y otras medidas básicas de prevención, los participantes en el año siguiente tuvieron una menor incidencia (1/278 = 0,4%; p <0,001). Las medidas de prevención básica puede reducir el riesgo de LC en los militares desplegados.
