Intercambiabilidad de biológicos de referencia a biosimilares en pacientes con enfermedades autoinmunes

Abstract

El uso de productos biológicos es cada vez más frecuente en el tratamiento de enfermedades con alto grado de complejidad como las autoinmunes o el cáncer, las cuales generan un alto gasto económico para el paciente y en los sistemas de salud. Los productos biosimilares son productos biológicos altamente similares a un producto biológico de referencia, y no tienen diferencias clínicamente significativas en términos de calidad, eficacia, seguridad e inmunogenicidad respecto a su referencia. El Perú cuenta con biosimilares aprobados, y las solicitudes de nuevos registros continuan en ascenso. A nivel local e internacional se tiene regulaciones de estos productos; sin embargo, respecto a la intercambiabilidad y el manejo del Switching (“Cambio”) y sustitución no existe una posición uniforme. Solo FDA cuenta con lineamientos para demostrar la intercambiabilidad, y a la fecha tiene autorizado dos biosimilares intercambiables. En el Perú la sustitución se aplica a nivel del sector público principalmente, donde la única opción de tratamiento son los biosimilares. Las sociedades médicas y farmacéuticas en su mayoría apoyan el cambio dirigido por el médico con el consentimiento del paciente, la sustitución automática no se recomienda. Es este contexto, esta investigación tiene por objetivo revisar las regulaciones, recomendaciones o posicionamientos sobre el intercambio de los productos biológicos por biosimilares o viceversa para el tratamiento de pacientes con enfermedades autoinmunes.

The use of biological products is increasingly more frequent in the treatment of highly complex diseases such as autoimmune diseases or cancer, which generate high economic costs for the patient and for health systems. Biosimilar products are biological products that are highly similar to a reference biological product, and do not have clinically significant differences in terms of quality, efficacy, safety and immunogenicity with respect to their reference. Peru counts with approved biosimilars, and the requests for new registrations continue to rise. At the local and international level, there are regulations for these products; however, regarding the interchangeability and management of Switching and substitution, there is no uniform position. Only the FDA has guidelines to demonstrate interchangeability, and to date it has authorized two interchangeable biosimilars. In Peru, substitution is applied mainly at the public sector level, where the only treatment option is biosimilars. Medical and pharmaceutical societies mostly support the change directed by the doctor with the consent of the patient, automatic substitution is not recommended. In this context, this research aims to review the regulations, recommendations or positions on the exchange of biological products for biosimilars or viceversa for the treatment of patients with autoimmune diseases.
