Identificación y Antibiograma Directo del Frasco Hemocultivo

Abstract

Introducción Los protocolos actuales de Laboratorio para bacteriemias requieren resembrar los frascos positivos para obtener colonias en medios sólidos y luego realizar las pruebas de identificación y sensibilidad antibiótica. Se evalúa la realización directa a partir del frasco hemocultivo positivo. Metodología Proceso Directo: extraer 6 ml del Frasco hemocultivo positivo a bacilos Gram negativos y en un Tubo con Gel Separador, centrifugar a 1000 g por 10 minutos, decantar completamente, agregar 0.5 mL de solución salina estéril y resuspender suavemente las bacterias sedimentadas sobre el gel. Preparar el inóculo McFarland requerido por el sistema Vitek2. Resultados 2,162 frascos hemocultivos entre Noviembre 2017 y Julio 2018. Se obtuvo 258 positivos (115 CGP, 118 BGN, 21 Levaduras, 2 Anaerobios). Se realizó ID y ATB directa a 43 cepas de BGN. Se obtuvo 95% de concordancia en ID (41/43): E.coli (11/12), E.cloacae (8/8), K.pneumoniae (6/6), Salmonella (6/6), P.aeruginosa (3/3), C.freundii (2/2), S.marcescens (1/1), M.morganii (1/1), B.cepacia (1/1), P.putida (1/1), A.baumanii (1/1) y A.lwoffii (0/1), la Identificación se obtuvo en 5.8 horas (3.75-10.25). En la sensibilidad antibiótica se tuvo 2 errores mayores y 4 errores menores, la concordancia en categoría de susceptibilidad fue de 99%: BLEE (8/8), Ampicilina/Sulbactam (36/36), Piperacilina/tazobactam (39/41 ME=1, MiE=1), Cefuroxima (36/36), Cefotaxima (42/43 MiE=1), Ceftriaxona (43/43), Ceftazidima (43/43), Cefepime (42/43 ME=1)), Ertapenem (36/36), Imipenem (43/43), Meropenem (43/43), Amikacina (43/43), Gentamicina (43/43), Ciprofloxacina (41/43 Mie=2), Tigeciclina (40/40), Trimetoprim Sulfametoxazol (40/40). Se obtuvo el informe de sensibilidad antibiótica en 11.1 horas en promedio (7.25 - 14.25). Conclusiones El uso del sistema de Hemocultivos Bactec FX, frascos Plus Aerobic/F, Plus Anaerobic/F, Ped Plus/F y el sistema Vitek® 2 con las tarjetas GN y AST-N249 directamente de los frascos positivos muestra concordancia con el método estándar igual o mayor al 95% para identificación y la sensibilidad antibiótica.
