Cáncer de tiroides en relación a tirotropina, insulinoresistencia y estado de autoinmunidad Hospital Edgardo Rebagliati Martins 2012-2013

Abstract

La razón de esta tesis fue establecer una relación de riesgo de malignización de un nódulo tiroideo hacia cáncer con marcadores bioquímico hormonales. El objetivo fue determinar la asociación de la tirotropina (TSH), los anticuerpos antitiroideos: antitiroglobulina (AbTg) y antitiroperoxidasa (AbTPO), y la resistencia a Insulina según método HOMA con el riesgo de presentar cáncer de tiroides en pacientes tiroidectomizados en el Hospital Edgardo Rebagliati Martins entre los años 2012-2013. El estudio tuvo una cohorte de 604 pacientes operados por bocio nodular, cumplieron los criterios de inclusión 337 pacientes, 158 casos (cáncer de tiroides) y 179 controles (patología benigna). Se realizó un estudio de casos y controles en una cohorte anidada. Paquete estadístico STATA 14.0, se usó test de chi cuadrado (p<0.005) para variables cualitativas, T exacta de Fisher (p<0,001) para las numéricas, estudio bivariado y multivariado con Regresión Login. OR con IC 95% para realizar ajuste epidemiológico y estadístico. Los resultados son: los niveles de TSH por encima de 1.5mUI/L se asociaron a mayor riesgo de presentar cáncer de tiroides OR 11,17 IC 95%, Ab-Tg se asoció a mayor riesgo de cáncer 2.7 con IC 95%.Proceder de la sierra tuvo un riesgo 3.3 veces mayor IC 95%. Ab-TPO se asoció a menor riesgo de presentar cáncer 0.34 IC 95%, como la edad mayor de 60 años. Las conclusiones son: Tirotropina (TSH), los Ab-Tgs y proceder de la sierra son factores de riesgo para presentar cáncer de tiroides bien diferenciado. Los Ab-TPOs fueron factores protectores para el cáncer de tiroides bien diferenciado.
