Abstract

La Malattia di Legg-Calvé-Perthes (LCPD) è una patologia dell’infanzia caratterizzata da necrosi avascolare transitoria della testa femorale, che può determinare deformità articolari e limitazioni funzionali a lungo termine. Nonostante i progressi in ambito ortopedico, la gestione conservativa rappresenta tuttora un approccio centrale, soprattutto nelle fasi iniziali e nei casi meno severi. La presente tesi si configura come una revisione narrativa della letteratura e ha l’obiettivo di analizzare le principali strategie conservative applicate nella LCPD, con particolare attenzione ai loro effetti clinici e radiografici. La ricerca bibliografica, condotta su PubMed e altre banche dati, ha selezionato studi degli ultimi dieci anni che valutano protocolli di trattamento basati su fisioterapia, limitazione del carico, ortesi e supporto farmacologico. I risultati emersi evidenziano come un trattamento personalizzato, che integri esercizi mirati al mantenimento della mobilità articolare, strategie di contenimento della deformità e un attento monitoraggio radiologico, possa migliorare gli outcome funzionali e ridurre il rischio di complicanze a lungo termine. In conclusione, la gestione conservativa della LCPD si conferma un’opzione efficace in molteplici contesti clinici, ma permane la necessità di ulteriori studi comparativi e di lungo follow-up per definire protocolli standardizzati e ottimizzare la prognosi dei pazienti affetti.
