Interdisciplinary Management of an Impacted Lateral

Abstract

An impacted tooth organ is one whose eruption is obstructed by a physical barrier, which can be bone, soft tissue or an associated pathology. Establishing an appropriate treatment plan in such cases usually involves holding an inter-consultation and providing interdisciplinary management. The report presents a 17-year-old female patient who came for an appointment because of the permanence of a deciduous tooth in the anterior superior sector of her mouth. Clinical examination revealed the persistence of dental organ 52; radiography showed a radiopaque lesion with a radiolucent edge compatible with odontoma. The treatment plan consisted of the hygienic phase, extraction of deciduous dental organ 52, enucleation of the odontoma and orthodontic traction of the impacted tooth to align it with the occlusal plane. In a control visit, mucogingival surgery was proposed to correct aesthetic defects following orthodontic traction. The aim in presenting this case is to describe an interdisciplinary approach to a retained lateral in the maxilla.

Resumen. Un órgano dentario impactado es aquel cuya erupción es impedida por una barrera física, que puede ser hueso, tejidos blandos o patología asociada. Establecer un plan de tratamiento adecuado ante estos casos implica realizar interconsulta y dar un manejo interdisciplinario. Se presenta a consulta paciente femenina de 17 años de edad, por la permanencia de un diente deciduo en boca en el sector anterosuperior. En el examen clínico se observó la persistencia de órgano dentario 52, radiográficamente se observó lesión radiopaca con borde radiolúcido compatible con odontoma. Como plan de tratamiento se realizó fase higiénica, extracción del órgano dentario deciduo 52, enucleación del odontoma y tracción ortodóntica del diente impactado para alinearlo en el plano oclusal. En una cita de reevaluación se propuso una cirugía mucogingival para corregir defectos estéticos posterior a la tracción ortodóntica. El objetivo del presente caso es la descripción del abordaje interdisciplinario de un lateral retenido en el maxilar superior.

Um órgão dentário impactado é aquele cuja erupção é impe-dida por uma barreira física, que pode ser osso, tecidos moles ou pa-tologia associada. Estabelecer um plano de tratamento adequado ante esses casos implica realizar interconsulta e dar um manejo interdisci-plinar. Apresenta-se na consulta paciente feminina de 17 anos de idade pela permanência de um dente decíduo em boca no setor anterossupe-rior. No exame clínico, observou-se a persistência de órgão dentário 52; radiograficamente se observou lesão radiopaca com borda radiolúcida compatível com odontoma. Como plano de tratamento, realizou-se fase higiênica, extração do órgão dentário decíduo 52, enucleação de odon-toma e tração ortodôntica do dente impactado para alinhá-lo no plano oclusal. Em uma consulta de reavaliação, propôs-se uma cirurgia muco-gingival para corrigir defeitos estéticos posterior à tração ortodôntica. O objetivo do presente caso é a descrição da abordagem interdisciplinar de um lateral retido no maxilar superior.
