Calcifilaxis: actualización y serie de casos

Abstract

Calciphylaxis is a complex disease that is triggered due to the imbalance and interaction between different conditions and variables, most of them present in patients with advanced kidney disease, although not exclusively. And it is characterized by the appearance of subcutaneous lesions that will evolve towards very painful necrotic ulcers. We present data from 4 patients with calciphylaxis, all women, with a mean age of 67.2 years with renal disease undergoing hemodialysis or peritoneal dialysis. The lesions of all of them occurred with necrotic ulcer, as well as areas of livers and nodular lesions. The diagnosis was confirmed by biopsy in all cases. Cure in wet environment and cutting debridement in all patients. In one case, surgical debridement and negative pressure therapy were included. The evolution was poor, the death of all patients occurred in an average time since the diagnosis of 1.3 months. Despite the uncertainties of calciphylaxis in terms of its pathogenesis, diagnosis, treatment and evolution, it seems necessary to establish central pillars in the treatment of the wounds that each patient will inevitably suffer, setting as a priority the prevention of infection, management pain and comfort.
