Tratamiento de la axila positiva tras terapia neoadyuvante

Abstract

El uso de la técnica biopsia selectiva de ganglio centinela está sustituyendo a la cirugía de vaciamiento axilar tras la terapia sistémica primaria. Su aplicación está aceptada en pacientes diagnosticadas inicialmente como cN0, en cambio persiste un debate en el caso de pacientes diagnosticadas con afectación axilar clínicamente positiva (cN+). El objetivo principal de esta revisión es identificar qué pacientes cN+, tras recibir tratamiento neoadyuvante podrían no necesitar linfadenectomía axilar, estudiando los criterios que predicen una respuesta patológica completa y tras analizar su impacto en los eventos oncológicos posteriores y su supervivencia.

The use of the sentinel lymph node biopsy technique is replacing axillary dissection surgery after primary systemic therapy. Its application is accepted in patients initially diagnosed as cN0; however, a debate persists in the case of patients diagnosed as cN+. The main objective of this review is to identify which cN+ patients, after receiving neoadjuvant treatment, may not require axillary lymphadenectomy, studying the criteria that predict a complete pathological response and analyzing their impact on subsequent oncological events and survival.
