Manejo endovascular de la hemorragia postparto

Abstract

Fundamentos y objetivos: La hemorragia postparto es una emergencia obstétrica considerada como la principal causa de mortalidad materna en el mundo. La embolización arterial es uno de los tratamientos empleados para el abordaje de dicha complicación. Se trata de una técnica mínimamente invasiva, preservadora de la fertilidad y con altas tasas de éxito registradas. El objetivo de este trabajo es revisar la literatura sobre del manejo endovascular de la hemorragia postparto con el fin de profundizar y contrastar los conocimientos disponibles acerca de la técnica. Material y métodos: Revisión sistemática de la literatura en la base de datos PubMed desde enero de 2016 a diciembre de 2022 seleccionando los artículos más relevantes según criterios de inclusión y exclusión. Resultados: fueron seleccionados 6 artículos para esta revisión. En los artículos de esta revisión se emplea mayoritariamente material embólico temporal como agente hemostásico. Se detectó extravasación de contraste por angiografía en el 71’01% de los casos. La arteria uterina es la más frecuentemente embolizada (59’9%). No se describen complicaciones en el 72’22% de los casos. Cuando aparecen las complicaciones menores (9’61%) son más frecuentes que las mayores (8’54%). La tasa de éxito de la técnica es 89’90%. Conclusiones: La técnica de embolización arterial es un método efectivo y seguro de abordaje de la hemorragia postparto con altas tasas de éxito y mínimas complicaciones.

Background and objectives: Postpartum hemorrhage is an obstetric emergency considered the main cause of maternal mortality in the world. Arterial embolization is one of the treatments used to address this complication, it is a minimally invasive, fertility preserving technique and with high success rates recorded. The objective of this work is to review the literature on the endovascular management of postpartum hemorrhage in order to deepen and contrast the available knowledge about the technique. Material and methods: Systematic review of the literature in the PubMed database from January 2016 to December 2022, selecting the most relevant articles according to inclusion and exclusion criteria. Results: 6 articles were selected for this review. In the articles of this review, temporary embolic material is mostly used as a hemostatic agent. Contrast extravasation was detected by angiography in 71.01% of cases. The uterine artery is the most frequently embolized (59.9%). No complications are described in 72.22% of cases, when minor complications appear (9.61%) they are more frequent than major ones (8.54%). The success rate of the technique is 89.90%. Conclusions: The arterial embolization technique is an effective and safe approach to postpartum hemorrhage with high success rates and minimal complications.
