Abstract

INTRODUCCIÓN: La puesta en marcha de estrategias para mejorar la detección de hipoglucemias en diabéticos con hipoglucemias inadvertidas es fundamental ya que las hipoglucemias inadvertidas aumentan el riesgo de sufrir hipoglucemias graves lo que supone un riesgo vital, repercute negativamente en la calidad de vida de los pacientes y muestran una mayor tendencia de complicaciones. OBJETIVO: Conocer la tasa de pacientes con hipoglucemia inadvertida entre los pacientes diabéticos que solicitaron atención urgente en un servicio de urgencias mediante la aplicación del test de Clarke. MATERIAL Y MÉTODOS: Se realizó un estudio observacional descriptivo prospectivo a 53 pacientes diabéticos que solicitaron atención por cualquier motivo en el Servicio de Urgencias del Hospital Universitario de La Ribera (SUHLR) en el periodo comprendido entre el 1 de enero y el 28 de febrero de 2020. Las variables a estudio fueron: sexo y edad del paciente, tratamiento previo para la DM, reconocimiento de la sintomatología ante la presencia de hipoglucemia, frecuencia de hipoglucemia grave sin pérdida de conocimiento en los últimos 6 meses, frecuencia de hipoglucemia grave en el último año, frecuencia de lecturas inferiores a 70 mg/dl en el último mes con y sin síntomas, cifras a partir de las cuales empieza a notar la sintomatología y percepción por los síntomas que su azúcar en sangre está bajo. RESULTADOS: La tasa de pacientes con hipoglucemia inadvertida fue del 5,66%. El 41,5% de los entrevistados refirió no tener ya síntomas cuando su nivel de azúcar en sangre está bajo. El 96,2% no había perdido la sintomatología que solía presentar ante una bajada de azúcar. Un 11,3% manifestó tener una o más veces al mes episodios de hipoglucemia grave sin pérdida de conocimiento en los últimos 6 meses, y un 15,1% haber tenido en el último año episodios de hipoglucemia grave. La frecuencia de lecturas inferiores a 70mg/dl con y sin síntomas fue respectivamente del 45,28% y del 5,66%. La cifra a partir de la cual indican mayoritariamente (81,1%) tener síntomas fue de 60-90 mg/dl. Solo un 9,4% señaló poder decir siempre por sus síntomas que su azúcar en sangre es bajo. CONCLUSIONES: Los Servicios de Urgencias de los Hospitales deben poner en marcha estrategias, como la aplicación del test de Clarke, para mejorar la detección de hipoglucemia en diabéticos con hipoglucemias inadvertidas. Estas estrategias deben llevarse a cabo a nivel multidisciplinar.

INTRODUCTION: The implementation of strategies to improve the detection of hypoglycaemia in diabetic patients with inadverten hypoglycaemia is essential since inadvertent hypoglycaemia increases the risk of severe hypoglycaemic attacks, which has a negative impact in the quality of life of the patients and shows a greater tendency for developing complications. OBJECTIVE: To acknowledge the rate of patients presenting with inadvertent hypoglycaemia among patients with Diabetes Mellitus who requested urgent care in the emergency room using Clarke test. MATERIAL AND METHODS: A prospective descriptive observational study was performed to 53 patients presenting Diabetes Mellitus who requested care of any reason to the Emergency Department of La Ribera University Hospital between 1st January and 28th February 2020. The variables used were: sex and age of the patient, previous treatment for DM, acknowledge of hypoglycaemia symptomatology, frequency of severe hypoglycaemic attacks without fainting during the previous 6 months, frequency of severe hypoglycaemic attacks during the last year, frequency of results inferior to 70 mg/dl during the last month with and without symptoms, values from which the patient starts having and perceiving the clinical presentation of low blood glucose. RESULTS: Rate of inadvertent hypoglycaemia was 5, 66%. 41, 5% had not symptoms while having low-blood glucose levels. A 96, 2% had the same symptoms they used to present with a drop in the glucose blood levels. A 11,3% reported having one or more times a month episodes of hypoglycaemia without loss of conciousness during the past 6 months, and a 15,1% reported having severe hypoglycaemic attacks in the past year. The frequency of readings inferior to 70mg/dl with and with no symptoms was respectively 45, 28% and 5, 66%. The value from which the patients mainly reported (81, 1%) having symptoms was of 60-90 mg/dl. Just a 9, 4% indicated to be certain about the correlation between blood glucose levels and the symptomatology. CONCLUSIONS: The Emergency Department should implement strategies, like Clarke test application, to improve the detection of inadvertent hypoglycaemia. These strategies should be done by a multidisciplinary team.
