Vivencias del proceso de transición al alta: perspectiva de enfermeras y pacientes

Abstract

La tendencia al envejecimiento de la población mundial, pone de manifiesto la prevalencia de diversas comorbilidades, dejando al descubierto la complejidad y fragilidad de las personas mayores. Dentro de este perfil, existe una mayor probabilidad de reingreso en períodos cortos de tiempo por descompensaciones clínicas. Con el fin de evitarlo, se ha observado que, dirigir las líneas de trabajo hacia una transición segura al alta, disminuye el número de reingresos. De ahí que se pretenda implantar una cultura hospitalaria eficiente con modelos estructurados de transición. Por este motivo que se considera necesario explorar la información que se transmite a los pacientes y sus necesidades con tal de facilitar una transición segura y favorecer la intervención y soporte de los profesionales de enfermería.
