As técnicas cirúrgicas na reparação das fendas palatinas: Uma revisão sistemática

Abstract

Introdução: As fendas faciais são os defeitos craniofaciais congénitos mais comuns nos seres humanos e constituem uma grande preocupação pessoal e social. A correção cirúrgica é o tratamento padrão para restaurar a anatomia e função normais da fenda palatina. Porém, a reparação de uma fenda é tecnicamente difícil. Os pontos fortes e fracos das várias técnicas de palatoplastia em termos de resultados são continuamente debatidos. Objetivo: O objetivo desta revisão é analisar a técnica de encerramento do palato associada aos melhores resultados e a uma baixa taxa de complicações. Material e método: Para tal, foi realizada uma pesquisa bibliográfica em bases de dados electrónicas como PubMed e ScienceDirect. Resultados: Foram seleccionados 24 dos estudos mais relevantes e foi efetuado um controle de qualidade dos mesmos. Discussão: Várias técnicas são apresentadas e comparadas entre si, das técnicas antigas às mais recentes. Foram analisadas as taxas de complicações causadas por elas. Conclusão: A palatoplastia de Furlow e a palatoplastia de Sommerlad-Furlow modificada parecem ter alcançado os melhores resultados nesta fase, embora novas técnicas pareçam promissoras para o futuro, mas requerem mais investigação.

Introduction: Facial clefts are the most frequent congenital craniofacial malformations in humans beings and are a major personal and social concern. Surgical correction is the standard treatment for restoring normal cleft palate anatomy and function. However, repairing a cleft is technically difficult. The strengths and weaknesses of the various palatoplasty techniques in terms of results are continually debated. Objective: This review purpose is to study the palate closure technique associated with the best results and a low complication rate. Material and method: Thus, a bibliographical survey was conducted in electronic databases such as PubMed and ScienceDirect. Results: 24 of the most relevant studies were selected and quality controlled. Discussion: Various techniques are presented and compared, from the old to the latest. Their complication rates are analysed. Conclusion: Furlow palatoplasty and modified Sommerlad-Furlow palatoplasty seem to have achieved the best results at this stage, although new techniques seem promising for the future, but require further research.
