Abstract

As lesões de furca ocorrem quando a doença periodontal atinge a zona da furca dos dentes multirradiculares causando a destruição óssea e perda de inserção no espaço interradicular. Diversos tipos de procedimentos e técnicas têm sido propostos para o tratamento das lesões de furca. A opção da técnica de tratamento pelo clínico deve ser sempre baseada no diagnóstico e classificação das lesões, bem como na capacidade de o paciente manter a região livre da acumulação de biofilme. O diagnóstico, avaliação e prognóstico correto são fundamentais para o estabelecimento do adequado tratamento periodontal das lesões de furca, afetando diretamente o curso e o sucesso da terapia. Até aos dias de hoje o tratamento das lesões de furca ainda é um desafio na clínica odontológica. As terapias regenerativas vieram alavancar forte inovação, proporcionando resultados promissores tanto em eficácia, como em durabilidade e segurança do tratamento de lesões da furca. O principal objetivo da regeneração periodontal de defeitos de furca é restaurar a autonomia e a função do periodonto danificado, o que engloba a criação de novo osso na base do defeito, bem como cemento e ligamento periodontal. Em suma, pretende-se regenerar o tecido perdido devido à periodontite.

Furcation lesions occur when periodontal disease reaches the furcation area of multirooted teeth causing bone destruction and loss of attachment in the interradicular space. Several types of procedures and techniques have been proposed for the treatment of furcation lesions. The clinician's choice of treatment technique should be based on the diagnosis, classification of the lesions and the patient's ability to keep the region free of biofilm accumulation. Correct diagnosis, assessment and prognosis are crucial for establishing adequate periodontal treatment of furcation lesions, they directly affect the course and success of therapy. Until today, the treatment of furcation lesions remains a challenge in clinical dentistry. Regenerative therapies have enabled strong innovation, providing promising results both in terms of efficacy, durability, and safety in the treatment of furcation lesions. The main goal of periodontal regeneration of furcation defects is to restore the autonomy and function of the damaged periodontium, which encompasses the creation of new bone at the base of the defect, as well as cementum and periodontal ligament. This means that the goal is to regenerate tissue that has been lost due to periodontitis.
