Abstract

A Fibromialgia é uma doença crónica musculoesqueletica de origem idiopática que se manifesta principalmente em mulheres na idade adulta, com prevalência mundial de cerca 5%. É caracterizada por dor difusa nos quatros quadrantes do corpo exacerbada após pressão nos pontos-gatilho. Juntamente com a dor muscular, sintomas como astenia, ansiedade, desordens do sono e depressão podem estar presentes, alterando significativamente a qualidade de vida. Esta revisão sistemática integrativa tem como objetivo analisar as opções terapêuticas disponíveis para a gestão da dor e das principais manifestações orofaciais que coexistem com a Fibromialgia. Uma revisão sistemática integrativa foi realizada a partir de uma pesquisa electrónica de artigos na língua inglesa publicados na base de dados da PubMed. Foi utilizada a seguinte combinação de termos: ((fibromyalgia) AND (temporomandibular disorders OR myofascial pain OR oral)), no período temporal dos 10 últimos anos, selecionando 17 artigos científicos relevantes sobre o tema. Os resultados apresentados descrevem uma alta prevalência de desordens temporomandibulares e queixas orofaciais nos pacientes fibromiálgicos. Foi observada dor miofascial, dor a palpação nos pontos-gatilho e dor durante os movimentos mandibulares. Queixas mastigatórias, dor de cabeça, estalidos na ATM e xerostomia também foram relatadas. Vários tratamentos farmacológicos e não farmacológicos foram descritos como eficazes na analgesia da dor e melhoria da qualidade de vida. Existe uma forte relação entre Fibromialgia e DTM, por esta razão o médico dentista deve ser capaz de reconhecer a sintomatologia da síndrome permitindo uma gestão terapêutica multidisciplinar.

Fibromyalgia is a chronic musculoskeletal disease of idiopathic origin that manifests itself mainly in women in adulthood, with a worldwide prevalence of about 5%. It is characterized by diffuse pain in the four quadrants of the body, exacerbated after pressure on the trigger point. Along with muscle pain, symptoms such as asthenia, anxiety, sleep disorders and depression can be present, altering significantly the quality of life of patients. This integrative systematic review aims to analyze the therapeutic options available for pain management and the main orofacial manifestations that coexist with Fibromyalgia. An integrative systematic review was carried out based on an electronic search for articles in English, published in the PubMed database. The following combination of terms was used: ((fibromyalgia) AND (temporomandibular disorders OR myofascial pain OR oral)), in the time period of the last 10 years, selecting 17 relevant scientific articles on this topic. The results presented describe a high prevalence of temporomandibular disorders and orofacial complaints in patients with fibromyalgia. Myofascial pain, pain on palpation at trigger points and pain during jaw movements were observed. Masticatory complaints, headache, TMJ clicks and dry mouth were also reported. Various pharmacological and non-pharmacological treatments have been described as effective in pain analgesia and improving quality of life. There is a strong relationship between fibromyalgia and TMD, for this reason, the dentist must be able to recognize the symptoms of this syndrome, allowing a multidisciplinary therapeutic management.
