In the last decade there have been dramatic advances in the understanding of genetic, biochemical and clinical aspects of disorders of the mitochondrial respiratory chain. Neurological manifestations are prominent although involvement of other systems is increasingly recognized. This article reviews current understanding of mitochondrial disease and in particular its genetic basis.
