Abstract

La finalidad de este estudio de caso interpretativo fue identificar las dificultades vividas por un equipo de profesionales que actúa en una Unidad de Tratamiento Intensivo. La compilación de datos fue realizada a través de observación participante y entrevistas semi-estructuradas llevadas a cabo con médicos, enfermeros y auxiliares de enfermería. El análisis reveló las siguientes categorías: cuidar en el tratamiento intensivo: "la ardua tarea de lidiar con la muerte y informar a la familia del paciente" y cuidar en el tratamiento intensivo: "la escasez de recursos materiales y la dificultad de trabajar en equipo". Las dificultades mencionadas se refieren al contacto con la familia, al afrontamiento de la muerte, a la falta de recursos materiales y especialmente a la relación entre los integrantes del equipo. Por consiguiente, el equipo debe promover discusiones y reflexionar sobre los aspectos técnico-científicos y éticos de la atención, tanto a los pacientes en estado crítico como de la familia de los pacientes, ante una mejor calidad de la atención y de las relaciones interpersonales.

Trata-se de um estudo de caso com uma abordagem interpretativa, cujo objetivo foi identificar as dificuldades vivenciadas pela equipe multiprofissional que atua na Unidade de Terapia Intensiva. Os dados foram coletados por meio de observações participantes e entrevistas semi-estruturadas com médicos, enfermeiros e auxiliares de enfermagem. Por meio da análise de dados, foram identificadas as categorias: cuidar em terapia intensiva - "é difícil lidar com a morte e informar a família", e cuidar em terapia intensiva - "a falta de recursos materiais e o difícil trabalho em equipe". As dificuldades relatadas estão relacionadas ao contato com os familiares, com o lidar com a morte, com a falta de recursos materiais e, especialmente, com o relacionamento entre os membros da equipe. Os resultados evidenciaram a necessidade de que a equipe promova momentos para reflexão e discussão acerca dos aspectos técnicos, científicos e éticos referentes ao cuidado tanto dos pacientes críticos quanto de seus familiares, tendo em vista a melhoria da qualidade do atendimento e do relacionamento interpessoal.

This interpretative case study aimed to identify the difficulties experienced by the patient care team while working at the Intensive Care Unit. Data were collected through participant observation and semistructured interviews with doctors, nurses and nurse's aides. Two major categories emerged from the data analysis: Care delivery in intensive care - "it is difficult to deal with death and inform the patient's family" and care delivery in intensive care - "the lack of material resources and the difficulty to work in a team". The difficulties are related to the contact with family members, dealing with death and the lack of material resources, and particularly with the relation between team members. Therefore, teams need to promote opportunities and discussions to reflect on the technical, scientific and ethic aspects of care for critical patients as well as family members, with a view to better care quality and interpersonal relations.
