Ocular adnexal malignant tumors with eccrine differentiation represent a rare group of neoplasms with a great invasive capacity. Authors present clinical findings in an eighty five years old patient who developed a progressive painless induration of both eyelids in his right eye.Microscopic examination of biopsy specimen suggested a primary eccrine sweat glands carcinoma of the eyelids after a thorough search for tumors. We also describe the surgical technique used for this treatment, as well as the current supportive therapies for these kinds of tumors.
