Ratlarda deneysel parasetamol toksikasyonunda Folik asit`in koruyucu etkilerinin araştırılması

Abstract

Parasetamol (asetaminofen) veya N-asetil-p-aminofenol (APAP), sentetik non-opioid olan para-aminofenol türevi ve fenasetinin aktif bir metabolitidir. Yaygın olarak kullanılan analjezik ve antipiretik bir ilaçtır. Akut karaciğer hasarının sebepleri arasında ilk sıralarda yer alan parasetamolün serbest oksijen ve nitrojen radikalleri üretimine neden olmakta, karaciğer ve böbrekte oksidatif hasara yol açmaktadır. B grubu bir vitamin olan folik asit başlıca nöral tüp defektlerinde, homosistein kaynaklı ateroskleroz, tromboembolizm ve vasküler endotel hasarlar ile kanser ve nörolojik hastalıklarda kullanılmaktadır. Yapılan çalışmalarda, folik asitin serbest radikallere ve reaktif oksijen türlerine karşı etkili olduğu bildirilmiştir. Bu çalışmada parasetamol ile deneysel olarak toksikasyon oluşturulmuş sıçanlarda, oksidatif hasarın önlenmesinde folik asitin etkinliği araştırıldı. Çalışma kapsamında 40 adet dişi Sprague dawley rat kullanıldı ve ratlar kontrol, parasetamol, folik asit, parasetamol+folik asit ve parasetamol+N-asetilsistein olacak şekilde 5 gruba ayrıldı (n=8). Parasetamol, folik asit ve N-asetilsistein hayvanlara sırasıyla 3 g/kg/gün, 20 mg/kg/gün ve 150 mg/kg/gün dozda oral yolla verildi. İlaç uygulamasından 24 saat sonra ratlar ötenazi edildi. Kan örnekleri ile karaciğer ve böbrek dokularında süperoksit dismutaz (SOD), katalaz (CAT) ve myeloperoksidaz (MPO) aktiviteleri ile glutatyon (GSH) ve malondialdehid (MDA) seviyeleri analiz edildi. Ayrıca bazı karaciğer ve böbrek fonksiyon testleri değerlendirildi. Parasetamol grubuyla karşılaştırıldığında parasetamol+folik asit grubunda serum SOD aktivitesinin yüksek (P=0.001) ve MDA seviyesinin düşük (P=0.001) olduğu belirlendi. Doku örneklerinde parasetamol grubuyla karşılaştırıldığında parasetamol+folik asit grubunda SOD ve CAT aktiviteleri ile GSH seviyesinin (böbrek dokusu hariç) istatistiksel olarak yüksek, MDA seviyesi ve MPO aktivitesinin (karaciğer dokusu hariç) ise istatistiksel olarak düşük olduğu görüldü. Ayrıca parasetamol+folik asit grubunda alanin aminotransferaz, kreatinin, üre ve gama glutamil transferaz değerlerinin parasetamol grubuna göre anlamlı olarak azaldığı saptandı. Bu çalışma sonucunda; parasetamol toksikasyonuna bağlı olarak oluşan oksidatif hasara karşı, folik asitin koruyucu olduğunu göstermektedir. Paracetamol (acetaminophen) or N-acetyl-p-aminophenol (APAP) is an active metabolite of para-aminophenol derivative and phenacetyl, a synthetic non-opioid. It is a widely used analgesic and antipyretic drug. Among the causes of acute liver injury, paracetamol, which is at the top of the list, causes the production of free oxygen and nitrogen radicals and causes oxidative damage in liver and kidney. Folic acid, a vitamin B group, is primarily used in neural tube defects, homocysteine induced atherosclerosis, thromboembolism and vascular endothelial damage in cancer and neurological diseases. Studies have shown, that folic acid is effective against free radicals and reactive oxygen species. In this study, the efficacy of folic acid in the prevention of oxidative damage in rats which were experimentally toxicated with paracetamol was investigated. 40 female Sprague dawley rats were used and rats were divided into 5 groups as paracetamol, folic acid, paracetamol+folic acid and paracetamol N-acetylcysteine (n=8). Paracetamol, folic acid and N-acetylcysteine were administered orally at a dose of 3 g/ kg/day, 20 mg/kg/day and 150 mg/kg/day, respectively. 24 hours after drug administration, rats were euthanized. Superoxide dismutase (SOD), catalase (CAT) and myeloperoxidase (MPO) activities and glutathione (GSH) and malondialdehyde (MDA) levels were analyzed in blood and liver tissues. Some liver and kidney function tests were also evaluated. In the paracetamol+folic acid group, serum SOD activity was higher (P=0.001) and MDA level was lower (P=0.001). In tissue samples compared to paracetamol group, SOD and CAT activities and GSH levels (except kidney tissue) were statistically higher, MDA level and MPO activity (except liver tissue) were statistically lower in paracetamol+folic acid group. In addition, in the paracetamol+folic acid group, alanine aminotransferase, creatinine, urea and gamma glutamyl transferase values were significantly decreased compared to the paracetamol group. As a result of this study; Folic acid is protective against oxidative damage due to paracetamol toxicity.Key words: Folic acid, oxidant/antioxidant parameters, oxidative damage, paracetamol, rat.
