Disfonia e disfagia in paziente con Sindrome di Forestier

Abstract

La Sindrome di Forestier (DISH: Diffuse idiopathic skeletal hyperostosis) è una patologia degenerativa sistemica a carico delle entesi, caratterizzata da alterata ossificazione dei punti di inserzione di legamenti, tendini e capsule articolari. È una patologia di riscontro a livello autoptico nel 18% della popolazione occidentale; aumenta di frequenza con l’età e coinvolge prevalentemente il sesso maschile (2:1). Interessa soprattutto il rachide toracico, seguito dal distretto cervicale nel 75% dei casi. La diagnosi è prettamente radiologica (Rx e TC); l’eziopatogenesi, incerta, è da ascriversi al microtraumatismo cronico associato alle più comuni alterazioni metaboliche dell’età adulta. Le manifestazioni cliniche, nelle forme di più alto grado possono alterare le funzioni aerodigestive, portando a disfonia, severa dispnea o disfagia, compromettendo la normale mobilità del sistema osteomuscolare del distretto cervicofacciale. La maggioranza dei pazienti è asintomatica o presenta lieve limitazione nei movimenti della colonna o dolore articolare
