Abstract

La tecnica Tomografia Computerizzata TC è considerata una procedura ad “alta dose” e gli sviluppi tecnologici degli ultimi anni ed i conseguenti cambiamenti pratici hanno consolidato questa definizione. Inoltre anche dal punto di vista normativo rientra nelle cosiddette “pratiche speciali” dell'art.9 del Decreto Legislativo 187/00. In termini di dose efficace una TC del torace è equivalente a 400 radiografie del torace, mentre ne servono 500 per ottenere una dose equivalente ad una sola TC dell'addome. La dose assorbita dai tessuti in seguito ad esami TC infatti è tra le più alte in radiologia diagnostica: 10-100mSv ed oltre. Le stime sul rischio di cancro radioindotto elaborate dallo studio epidemiologico sui sopravvissuti ai bombardamenti atomici dimostrano un eccesso di rischio, piccolo ma statisticamente significativo, a dosi inferiori a 50 mSv e quindi comparabili a quelle associate a studi TC. L'ente britannico NRPB (National Radiation Protection Board) ha stimato un incremento pari a 1 su 2000 del rischio cumulativo a 70 anni di mortalità per tumore in seguito a un esame TC dell'addome. Di conseguenza i carichi di dose all'individuo ed alla popolazione associati agli esami TC sono attualmente regolamentati dalle comunità scientifiche ed autorità sanitarie in tutto il mondo. Nelle Aziende Sanitarie dell'Emilia-Romagna vengono annualmente eseguite circa 380.000 esami TC e la quota di dose efficace collettiva attribuibile a questa metodica è pari al 64% del totale; in Gran Bretagna è il 40%, negli USA il 67%. Tutte queste considerazioni, connesse alla rapidità con cui evolve la tecnologia, rendono necessaria una strategia di governo di questa pratica adottando misure che incidano sulla frequenza di esecuzione e sul mantenimento della dose ai livelli più bassi compatibili con la qualità dell'immagine diagnostica.
