Abstract

L’infezione da CMV rappresenta una delle più comuni infezioni opportuniste ad eziologia virale nei pazienti immunocompromessi. La malattia da CMV nel paziente si può presentare con un’ampia gamma di manifestazioni cliniche (Tabella 1), può rappresentare un fattore di elevato rischio per la compromissione degli organi e minacciare quindi la sopravvivenza dei pazienti. La quasi totalità dei pazienti trapiantati d’organo solido e di cellule staminali emopoietiche sviluppa un’infezione da CMV che solo nel 30-50% dei casi, in assenza di strategie di profilassi o di terapia presintomatica, ha ripercussione clinica di varia entità. Nei pazienti con AIDS invece, le patologie più frequenti sono la retinite e i sintomi gastrointestinali in particolar modo nei soggetti severamente immunodepressi. Sebbene l’incidenza della retinite da CMV si è notevolmente ridotta dopo l’introduzione dell’HAART (Highly Active Antiretroviral Therapy), nei pazienti con AIDS e retinite il rischio di mortalità è molto elevato come elevato è il rischio di insorgenza di complicazioni della retinopatia e perdita della vista.
