Abstract

Un gel di pilocarpina (PILOGEL®) è stato di recente introdotto sul mercato italiano e dai primi studi effettuati ha dimostrato una efficacia paragonabile per azione ipotensiva al prodotto in collirio al 2-4%. Per verificare quanto da altri Autori asserito, è stato intrapreso il presente studio volto a confrontare l'effetto della pilocarpina 4% gel Vs pilocarpina 2-4% collirio. Sono stati sottoposti a trattamento con Pilogel® pazienti affetti da glaucoma cronico semplice in precedenza già trattati con pilocarpina 2 e 4% collirio. Mentre il confronto pilocarpina 4% gel Vs pilocarpina 4% collirio non ha evidenziato differenze significative tra l'effetto ipotonizzante dei due farmaci ad eccezione delle prime ore pomeridiane in cui il collirio è significativamente più efficace, il confronto pilocarpina 4% gel Vs pilocarpina 2% collirio ha messo in evidenza risultati temporalmente opposti e cioè una maggiore efficacia ipotensiva della pilocarpina 4% gel la mattina e con assenza di variazioni significative nel pomeriggio.Per quanto riguarda l'andamento tonometrico nel corso della giornata dopo somministrazione di Pilogel® è stato osservato il massimo incremento pari a 0.93±1.96 mmHg tra le ore 10 e le ore 11, circa 12 ore dopo la somministrazione. Gli Autori concludono che l'effetto del Pilogel® può essere assimilato nel corso della mattinata a quello della pilocarpina 4% collirio e nel pomeriggio a quello della pilocarpina 2% collirio per l'incremento tonometrico statisticamente significativo (10% circa) riscontrato 12 ore dopo la somministrazione del farmaco (h. 22) e mantenutosi costante nel corso della rimanente giornata.
