Abstract

Le anemie aplastiche sono caratterizzate da un numero ridotto di tutte le linee cellulari e da assenza o diminuzione marcata dei rispettivi progenitori midollari. Si può anche osservare citopenia di una o due linee emopoietiche che possono rimanere tali o preludere alla pancitopenia. Sia la pancitopenia che la riduazione di una singola linea cellulare si distinguono in ereditarie o costituzionali e acquisite. Per molte delle forme ereditarie è stato dimostrato che il deficit è dovuto a mutazioni genetiche mentrei fattori acquisiti possono essere operativi di per sè o interagire con mutazioni genetiche per indurre varie espressioni cliniche della malattia. Sebbene sia difficile stabilire l'incidenza di tuitte le forme ereditarie, l'anemia di Fanconi viene considerata la forma più comune mentre la trombocitopenia amegacariocitica quella più rara. la prognosi di questa fome è migliorata negli ultimi anni grazie all'impiego di trapianto di midollo osseo da familiare o donatore HLA compatibile o da sangue di cordone ombelicale. Le anemie acquisite si distinguono in idiiopatiche, come gran parte dei casi in età pediatrica, oppure secondarie a numerose cause. Queste sono costituite da infezioni virali, in particolare il Parvovirus B19 e l'EBV, farmaci, sostanze tossiche, radiazioni. queste forme raramente vanno incontro a guarigione spontanea e se non trattate hanno una prognosi infausta a causa di infezioni o emorragie. Il trattamento si avvale di una terapia di supporto (antibiotici, trasfusione di emazie o piastrine) e di una terapia specifdica mediante trapianto di midollo osseo da donatore o familiare HLA compatibile
