Abstract

Per disturbi del comportamento alimentare (DCA), o disturbi dell’alimentazione (DA) si intende l’insieme di patologie psichiatriche concernenti grossolane alterazioni del comportamento alimentare. L’anoressia nervosa (AN) e la bulimia nervosa (BN) sono i disturbi del comportamento alimentare più importanti e più noti. L’esperienza clinica e la letteratura scientifica richiamano sempre più frequentemente l’attenzione su una terza categoria rappresentata dal disturbo da alimentazione incontrollata, che tuttavia non corrisponde ancora a una diagnosi psichiatrica generalmente condivisa. Tale disturbo è stato attualmente e provvisoriamente inserito all’interno di un ampio spettro di comportamenti alimentari che sono a tutt’oggi in corso di valutazione: i disturbi dell’alimentazione non altrimenti specificati (DANAS), descritti dai criteri di ricerca del Manuale diagnostico e statistico dei disturbi mentali (DSM). Sebbene siano conosciuti alcuni casi di anoressia descritti in epoche molto antiche, l’identificazione della malattia come entità clinica a sé stante risale al 1873: due medici, l’inglese Gull e il francese Lasègue, partendo da differenti esperienze e osservazioni approdarono alla medesima conclusione studiando un certo numero di giovani donne che inesorabilmente e a dispetto di qualsiasi tentativo di terapia, cessavano di alimentarsi, sopravvivevano per un certo periodo in stato di inedia ed infine morivano. Anche la bulimia nervosa condivide le origini remote dell’anoressia, ma la sua definizione e catalogazione nosografica si sono avute in epoca ancora più recente.
