Abstract

CAPITOLO 13 • 340 SISTEMA GASTROINTESTINALE 13.3 STIPSI E DIARREA Francesco Occhiuto Le funzioni intestinali sono regolate da un sistema piuttosto complesso (vedi Box 1.) I disordini della motilità rappresentano i più comuni disturbi intestinali. Essi variano dagli spasmi della muscolatura liscia, al transito intestinale rallentato, alla stipsi e alla diarrea. Sono note condizioni caratterizzate da ridotta motilità propulsiva e ipotonie del crasso che possono evolvere fino all’atonia e alla paresi dei movimenti intestinali (stitichezza atonica, megacolon). In queste evenienze la velocità di transito del contenuto intestinale rallenta e ciò comporta un aumento del tempo di permanenza del materiale fecale che, in ultima analisi, può esitare in un’occlusione intestinale. In questi casi, possono essere impiegati, per brevi periodi, alcuni farmaci lassativi in grado di incrementare i movimenti di massa del colon e facilitare lo svuotamento intestinale. Vi sono, anche, disturbi in cui la contrattura della muscolatura liscia intestinale, determina spasmi intestinali dolorosi con stitichezza spastica. Vi sono, inoltre, condizioni in cui, per l’ingestione di sostanze irritanti o di sostanze osmoticamente attive, o sostanze alimentari grasse non adeguatamente digerite e assorbite, o per infezioni, si verificano esaltazioni dei movimenti propulsivi intestinali, con diarrea (emissione di feci liquide) spesso accompagnata da dolori colici. In questi casi, si può far uso di farmaci ad azione antispastica ed antidiarroica.
