METASTASI DA CARCINOMA NEUROENDOCRINO DELLA CUTE. CASO CLINICO

Abstract

Sulla base della osservazione di un caso di metastasi da tumore neuro-endocrino della cute gli AA. analizzano le più recenti acquisizioni in termini diagnostico-terapeutici di questa rara neoplasia altamente aggressiva che si sviluppa in prevalenza nelle aree cutanee esposte al sole. L’esame istologico ed immunocitochimico di una voluminosa tumefazione di forma ovalare localizzata al cavo ascellare dx ha confermato la natura neuroendocrina delle lesioni (tumore a cellule di Merkel) considerandola come una localizzazione ripetitiva secondaria in sede cutanea di una formazione primitiva del braccio omolaterale. Il caso descritto ha confermato l’alta incidenza di metastatizzazione del tumore neuroendocrino primitivo il cui iter diagnostico-terapeutico risulta difficoltoso per il numero limitato di casi osservati ed il cui trattamento elettivo è la rimozione chirurgica con l’obiettivo della maggiore radicalità possibile, associata alla linfoadenectomia loco regionale; la radioterapia complementare può far ridurre le recidive locali migliorando la sopravvivenza a lungo termine.
