COLPOSACROPESSIA ADDOMINALE CON PRESERVAZIONE DELL’ UTERO: RISULTATI A LUNGO TERMINE

Abstract

Scopo del lavoro L’ Isterocolposacropessia (ICS) è uno degli approcci chirurgici al prolasso uterovaginale (PUV) severo. Questo studio valuta gli outcomes anatomici e funzionali a lungo termine (f-up minimo di 4 anni) dopo ICS per via addominale (ICSA). Materiali e metodi Da dicembre 1996 e maggio 2013 64 pz con PUV (II-IV stadio) sono state sottoposte ad ICSA, di queste sono state incluse 60 con f-up minimo di 4 anni. Valutazione pre-op: anamnesi, valutazione sintomi urinari e sessuali, incontinenza urinaria (IU), esame obiettivo (POPq system), uroflussometria (UF) con RPM, urodinamica, ecografia transperineale, Urinary Distress Inventory Short Form (UDI6), Incontinence Impact Questionnaire–Short Form (IIQ7), Female Sexual Function Index questionnaire (FSFI). Follow-up ad 1, 3, 6, 12 mesi e poi annuale con: esame obiettivo, valutazione sintomi urinari e sessuali, UF + RPM, Patient Global Impression of Improvement (PGI) UDI6, IIQ7 e FSFI a 1 e 2 anni. Outcomes valutati: anatomici, funzionali, sintomatologici, percezione globale del trattamento da parte delle pz. Analisi statistica: Mann-Whitney e X2 test. Studio approvato dal comitato etico locale Risultati Valutate 60 pz: età media 59,6±11 anni, BMI medio 24.7±4, parità mediana 2 (range 1- 5), follow-up mediano di 60 mesi (range 48-132). Tasso di successo anatomico: 100% per il prolasso apicale; 72,4% e 76,4% per compartimento anteriore e posteriore (recidive Discussione Gli outcomes confermano l’efficacia della ICSA nel trattamento del PUV. Il tasso di successo anatomico è dimostrato dalle basse persistenze di recidive, asintomatiche e stabili nel tempo. I sintomi urinari, IUU e IUS migliorano significativamente. I risultati sono confermati dall’alto score del PGI. Conclusioni Lo studio conferma gli ottimi outcomes funzionali e anatomici a lungo termine del ICSA.
