Abstract

Il termine tiroidite indica un gruppo eterogeneo di patologie infiammatorie della tiroide di diversa etiologia (69). Anche se il termine tiroidite evoca, per definizione, “tumor, rubor, dolor et functio laesa”, non sempre le tiroiditi causano ingrossamento e dolore tiroideo. La presentazione clinica varia, infatti, da forme con le caratteristiche tipiche dell’infiammazione nella regione tiroidea, a forme prive di sintomatologia locale che si rendono evidenti solo con i segni e i sintomi della disfunzione ghiandolare o, ancora, a forme asintomatiche che vengono diagnosticate per l’alterazione di alcuni parametri di laboratorio (13). L’etiologia è infettiva, autoimmune, da traumi, da radiazioni, da farmaci o da processi fibrotici idiopatici. Il decorso può essere acuto, subacuto o cronico; il processo infiammatorio può non influenzare la funzione tiroidea o, al contrario, può determinare tireotossicosi, ipotiroidismo o entrambe le condizioni in sequenza con successivo ripristino dell’eutiroidismo o evoluzione verso ipotiroidismo permanente.
