Abstract

La dermatite atopica (DA) è una malattia infiammatoria cronica della cute, la cui incidenza e prevalenza è in continuo aumento nei paesi industrializzati. Attualmente, la DA colpisce il 10-20% dei bambini e l’1-3% degli adulti. La DA è una malattia eterogenea che spesso si associa a rino-congiuntivite e asma bronchiale. La patogenesi è complessa con un ruolo fondamentale di fattori genetici con cui interagiscono una varietà di fattori ambientali. Entrambe le componenti condizionano l’eccessiva risposta TH2 verso gli allergeni e l'iperreattività infiammatoria cutanea. Le lesioni cutanee sono il risultato di risposte immuno- allergiche ma pure di un’esagerata reattività infiammatoria cutanea. A questa contribuisce un’alterata funzione della barriera epidermica, responsabile anche della marcata secchezza cutanea. L’approccio al paziente con DA deve prevedere il riconoscimento e Ia rimozione dei fattori ambientali scatenanti. La terapia della DA si avvale di un’adeguata cura della cute, dei corticosteroidi topici e degli inibitori della calcineurina topici. Il prurito è parzialmente controllato dagli antistaminici sistemici. Nei casi più severi, prevede l’impiego della fototerapia, di corticosteroidi e di farmaci immunosoppressivi sistemici.
