Abstract

Amaç: Sağlık bakım hizmetlerinin kişilere vereceği zararı önlemek amacıyla sağlık kuruluşları ve bu kuruluşlardaki çalışanlar tarafından alınan önlemlerin tamamı hasta güvenliğini oluşturmaktadır. Bu çalışmada hasta güvenliği ile ilgili çalışmaların sistematik olarak incelenmesi amaçlanmıştır. Gereç ve Yöntem: Konuyla ilgili 2004 ile 2014 tarihleri arasında yayınlanmış 5649 çalışma incelenmiştir. Araştırmaya dahil edilme kriterlerine uygun toplam 10 yayın çalışma kapsamına alınmış ve karşılaştırma, sınırlılık ve sonuçlar açısından değerlendirilmiştir. Bulgular: Sağlık personelinin %6.2’si tıbbi hata yaptığı, %10.4’ü arkadaşının yaptığı tıbbi hataya tanık olduğunu belirtmişlerdir. Cerrahi alan enfeksiyonu, ameliyat sonrası komplikasyonlar, iyileşme zamanının uzaması ve yatak yaralarının oluşması gibi komplikasyonlar ve (%41) uygun antibiyotik kullanılmaması gibi durumlara tıbbi hataların neden olduğu bildirilmiştir. Ameliyathanelerde iletişim eksikliği, bilgi eksikliği, artan iş yükü, hemşirelere görev dışı işlerin yüklenmesi, personel ihtiyacının karşılanmaması ve çalışanlar arasındaki rekabet gibi unsurların hasta güvenliği için büyük tehdit oluşturduğu belirtilmektedir. Yapılan çalışmalarda öğrenci hemşirelerin çalışan hemşirelere göre daha fazla tıbbi hatalara neden olduğu vurgulanmaktadır.Sonuç: Ameliyathanede hasta güvenliğinin sağlanmasının; uygun güvenli ergonomik koşulların oluşturulması, çalışanların bu konuda yeterli bilgi beceriye sahip olması ve uygun çalışma koşullarının sağlanması ile mümkün olacağı düşünülmektedir.

Aim: Patient safety is composed of all the precautions taken by the health institution sand their staff in order to prevent the harm that might be caused by health care services. In this study it is aimed to examine the studies related to patient safety systematically. Instrumentand Method: 5649 studies published between the years 2004 and 2014 have been examined and 10 publications which conform to inclusion criteria have been included in the study and evaluated in terms of comparison, limitation and results. Findings: 6.2% of the health personel stated to have made a medicalerror and 10.4 % of them stated to have witnessed a friend making a medicalerror. Surgical site infection, postoperative complications, extended recovery periods, bedsores as complications and (41%) were not using appropriate antibiotic as situation were reported that the cause of medical errors. Factors such as lack of communication in surgery rooms, lack of information, increasing work load, nurses doing extraduties, the need for personnel not being satisfied and competition among the staff were reported to pose a great danger for patient safety. Result: It is thought that providing patient safety in surgery rooms would only be possible by creating appropriate ergonomic facilities, staff having sufficient information and skill sand providing appropriate working conditions.
