Abstract

Hepatit virusları hemodiyaliz (HD) hastalarında en önemli infeksiyon problemlerinden biri haline gelmiştir. Hastalar arasında nozokomiyal geçişin kanıtları olmasına rağmen, diyaliz ünitelerinde hepatit C virus(HCV)unun kesin bulaşma şekli bilinmemektedir. Bu çalışmanın amacı Denizli' de HD hastalarında hepatit C virüs antikor prevalansını, risk faktörlerini saptamak ve bu hastalarda HCV infeksiyonunun bulaşmasının önlenmesinin tartışılmasıdır. 2 HD merkezinden toplam 155 hasta çalışmaya alındı.Anti-HCV 3. jenerasyon enzyme-linked immunosorbent assay(ELISA) yöntemiyle saptandı ve 155 HD hastasının 50 (%32.26)'sinde anti-HCV pozitif bulundu. Ortalama HD tedavi süresi anti-HCV pozitif hastalarda (48.7 ay) anti-HCV negatif hastalardan (27.3 ay) anlamlı olarak daha uzun bulundu (p=0.000). Anti-HCV pozitif hasta oranı HD tedavi süresi ile artmaktadır(p=00001). Bu çalışma HD hastalarının HCV infeksiyonu için risk taşıdığını ve HD tedavi süresi, anti-HCV pozitifliği arasındaki ilişkinin ünite içi bulaşmanın indirek kanıtı olabileceğini göstermiştir. HD hastalarında HCV'unun nozokomiyal bulaşma insidansını azaltmak için aseptik tekniklerin ve hijyenik önlemlerin çok dikkatli uygulanması zorunludur.
