Abstract

This study was planned to investigate of the treatment efficiency of manual treatment techniques in temporomandibular joint dysfunction.A total of 24 individuals ( age range: 20-50; 17 females , 7 males) were randomly divided into two groups ( 1. Study group: 12, 2. Study group: 12). All subjects received electrophysical agents (EFA) (infrared, TENS, ultrasound) and exercise therapy for 10 sessions (5 days a week). In addition to the individuals in the first study group , 6 sessions were performed with manual therapy techniques (MTT) (soft tissue mobilization, joint mobilization and friction massage) day after day. Assessments of pain severity (visual analogue scale), pressure pain threshold (digital algometer), cervical normal joint movement (CROM device), jaw movements (caliper), quality of life (Nottingham Health Profile), and mental status (Beck Anxiety Inventory) were performed before and after the treatment. Patients treatment satisfaction was assessed by using VAS (visual analogue scale) after treatment. Although MTT is effective in many other parameters, it has provided a base for increasing the functionality of the temporomandibular joint (TMJ) according to EFA.In the first study group, the severity of pain (except mouth closure), mouth opening and left lateral movement distance, all cervical range of motion (ROM), NSP and Beck Anxiety Inventory values were different after treatment (p <0.05).When the two groups were compared after treatment, the severity of chewing pain and the functional status of TMJ were significantly different in favor of the first group (p <0.05).The results of this study showed that conventional physiotherapy methods were an effective treatment modality in TMED. In addition to conventional physiotherapy, MTT has been shown to provide additional benefit in terms of pain intensit and functional status.Keywords: Temporomandibular joint, physiotherapy, TMJ functionality

Bu çalışma temporomandibular eklem disfonksiyonunda (TMED) konvensiyonel fizyoterapiye ek olarak uygulanan manuel tedavi tekniklerinin tedavi etkinliğinin incelenmesi amacıyla planlandı. Toplam 24 hasta (yaş aralığı 20-50; 17 kadın, 7 erkek ) randomize olarak iki gruba (1. Çalışma grubu: 12, 2. Çalışma grubu: 12) ayrıldı. Tüm hastalara elektrofiziksel ajanlar ve egzersiz tedavisi toplam 10 seans uygulandı. 1. Çalışma grubundaki hastalara ek olarak manuel tedavi teknikleri (MTT) gün aşırı olmak üzere 6 seans uygulandı. Tedavi öncesi ve sonrası ağrı şiddeti Görsel Analog Skalası ile (GAS), basınç ağrı eşiği dijital algometre ile servikal bölge normal eklem hareketi (NEH) CROM cihazı ile, mandibula hareketleri kaliper ile, fonksiyonel durum Kranio Mandibular İndeks ile, yaşam kalitesi Nottingham Sağlık Profili (NSP) ile ruhsal durum Beck Anksiyete Envanteri (BAE) değerlendirildi. Hastaların tedavi memnuniyeti tedaviden sonra GAS ile değerlendirildi. Birinci çalışma grubunda tedavi sonrası ağrı şiddeti (ağız kapama dışında), ağız açma ve sol lateral hareket mesafesi, tüm servikal bölge NEH, NSP ve BAE değerleri tedavi öncesine göre farklıydı (p<0.05). İkinci grupta ağrı şiddeti değerleri, TME hareketleri (ileri hareket dışında) ve NSP tedavi öncesine göre farklıydı (p<0.05). İki grup tedavi sonrası karşılaştırıldığında çiğneme sırasında ağrı şiddeti ve TME fonksiyonel durumu birinci grup lehine anlamlı derecede farklıydı (p<0.05)Bu çalışmanın sonuçları konvensiyonel fizyoterapi yöntemlerinin TMED' de etkili bir tedavi yöntemi olduğunu göstermiştir. Konvensiyonel fizyoterapiye ek olarak uygulananan MTT' nin ağrı şiddeti ve fonksiyonel durumda ek yarar sağladığı belirlenmiştir.Anahtar Kelimeler: Temporomandibular eklem, fizyoterapi, TME fonksiyonelliği
