Abstract

Amaç: Kronik Böbrek Hastalığının (KBH) tüm evrelerinde artmış oksidatif stres (OS) yükü saptanmaktadır. Hemodiyaliz (HD) hastalarının Sürekli Ayaktan Periton Diyaliz (SAPD) hastalarına göre daha yüksek risk taşıdıkları öne sürülmüştür. Bu çalışmada HD ve SAPD hastalarında OS ile ekokardiyografik indeksler üzerindeki etkileri incelenmiştir. Gereç ve Yöntem: Çapraz?kesitsel bir araştırma olarak planlanan çalışmaya 39 SAPD, 32 HD ve benzer demografik özellikleri taşıyan 30 sağlıklı gönüllü alındı. Diabetes Mellitus ve kronik inflamatuar hastalığı olanlar kapsam dışı bırakıldı. Oniki saatlik açlığı takiben hafta ortası diyaliz seansında alınan serumlarda, biyokimyasal, hormonal ve hematolojik parametrelerinin yanı sıra malonildialdehit (MDA), glutatyon peroksidaz (GSH-px) ve superoksit dismutaz (SOD) çalışıldı. M-mod ekokardiyografi kullanarak ejeksiyon fraksiyon (EF), interventriküler septum kalınlığı (IVSd), arka duvar çapı (LVPWd) ve sol atrium çapı (LAd) ölçülerek OS parametreleri ile karşılaştırıldı. Bulgular: Hasta ile kontrol grubu yaş, cins, beden kitle indeksi, kan basıncı yönünde farksızdı. MDA ve GSH-px her iki grup arasında anlamlı bulunmazken, SOD hasta grubunda kontrol grubuna göre belirgin düşük bulundu (p < 0.0001). HD ve SAPD hastaları arasında yaş, cins, kan basıncı, beden kitle indeksi, hemoglobin, C-reaktif protein, ferritin, kalsiyum, iPTH ve oksidatif stres parametrelerinden MDA ve GSH-px benzerdi. SOD düzeyleri istatistiksel olarak HD hastalarında daha düşük bulundu (p = 0.039). Serum fosfor, kalsiyum-fosfor çarpımı (CaxP), albumin HD hastalarında SAPD hastalarına göre daha yüksek; fibrinojen, total ve LDL kolesterol daha düşük bulundu. MDA ile EF arasında negatif bir ilişki (r=-0.380, p=0.001) saptandı; SOD ile sistolik kan basıncı (r=-0.265, p=0.011), diastolik kan basıncı (r=-0.230, p=0.028), fosfor (r=-0.327, p=0.001), iPTH (r=-0.259, p=0.013), CRP (r=-0.235, p=0.024), Fibrinogen (r=-0.342, p=0.001), T. kolesterol (r=-0.249, p=0.017) olumsuz birliktelik vardı. SOD ile hemoglobin (r=0.414, p<0.001) ve albumin (r=0.367, p<0.001) arasında olumlu bir birliktelik vardı. MDA yaş (beta=-0.258, p=0.035), erkek cinsiyet (beta=-0.312, p=0.004) ve EF (beta =-0.461, p<0.001) ile bağımsız olarak etkileştiği saptandı. Antioksidanlar ile kardiyak indeksler arasında herhangi bir korelasyon bulunmadı. Sonuç: HD ve SAPD tedavisi alan son dönem böbrek yetmezliği hastalarında normal popülasyona kıyasla OS parametrelerinden SOD'un hemodiyalizde daha belirgin olmak üzere azalarak OS yol açabileceği, MDA seviyesinin bağımsız olarak yaş, erkek cinsiyet, ve EF ile birliktelik gösterdiği; buna mukabil SOD kan basıncı, P, PTH, CRP ve fibrinojen ile negatif ve hemoglobin ve albumin ile pozitif korelasyon gösterdiği bulundu. Aim: Enhanced oxidative stres (OS) is present in all stages of chronic renal disease. It is suggested that hemodialysis (HD) patients are under higer risk than continous ambulatory peritoneal dialysis (CAPD) patients. In this study, the affects of OS on echocardiographic indexes in HD and CAPD patients were evaluated. Materials and Methods: 39 CAPD, 32 HD patients and 30 healthy volunters having similar demographics were included into this cross-sectional study. Who had Diabetes Mellitus and chronic inflammatory diseases were excluded. Biochemical, hematological parameters and malonyldialdehyte (MDA), glutathion peroxydase (GSH-Px), and superoxyde dismutase (SOD) were studied from sampels taken after overnight period and before mid-week dialysis session. EF, IVSd, LVPWd, and LAd were measured by M-Mode echocardiography. Results: There was no difference between patients and controls in age, gender, bodyBMI, and blood pressure. While no difference was found in MDA and GSH-Px between groups SOD was significantly lower than controls in patients (p<0.001). Age, gender, blood pressure, BMI, hemoglobin, C-RP, ferritin, calcium, iPTH, and MDA, and GSH-Px as oxidative stres parameters were similar among HD and CAPD patients. SOD levels were significantly lower in HD patients (p=0.039). Serum levels of P, albumin, and Ca-P product were higher, and serum levels of fibrinogen, total cholesterol, and LDL-Cholesterol were lower in HD patients than CAPD group. A negative corellation between MDA and EF (r=-0.380, p<0.001), and negative correlations were found between SOD and systolic blood pressure, diastolic blood pressure, phosphorus, iPTH, C-RP, fibrinogen, total cholesterol levels. Positive correlations were found between SOD and Hb, and albumin levels. MDA interacts with male gender, and EF independently. There was no correlation between antioxidants and cardiac parameters. Conculision: SOD levels as oxidative parameters may cause enhanced OS by decreasing significantly lower levels in end stage renal disease patients especially in whom undergoing HD treatment. MDA levels were correlated with age, male gender, and EF, while SOD levels had negative correlations with BP, P, iPTH, C-RP, and fibrinogen, and psitive correlations with Hb, and albumin levels.
