Vital pulpa tedavilerinde kullanılan materyallerin biyouyumluluğu

Abstract

Derin dentin çürüklü dişlerin vitalitesinin korunması, kök gelişiminin devamı için önemlidir. Vital pulpa tedavilerinde kullanılacak materyaller; mikrosızıntıyı önlemeli, tamir fonksiyonunu uyarmalı, antibakteriyel ve biyouyumlu olmalıdır. Günümüzde, vital pulpa tedavilerinde en çok; Kalsiyum hidroksit, MTA, Biodentin ve Bioaggregate gibi materyaller kullanılmaktadır. Kalsiyum hidroksit genellikle; pulpa kaplaması, pulpotomi, periapikal lezyon, internal rezorbsiyon ve apeksifikasyon vakalarında tercih edilen bir materyaldir. Bununla birlikte, kalsiyum hidroksite bağlı kronik iltihap ve nekroz oluşumundan söz edilmektedir. MTA’nın mikrosızıntıyı önlediği, pulpa ve periradiküler dokular ile biyouyumlu olduğu ve bu dokuların rejenerasyonunu uyardığı belirtilmektedir. MTA’nın manipülasyon güçlüğü, geç sertleşmesi ve kolay renklenmesi gibi dezavantajlarını ortadan kaldırmak üzere geliştirilen Biodentin; biyouyumluluğu ve antibakteriyel özellikleriyle MTA ve diğer trikalsiyum silikat içerikli materyallerden üstün özelliklere sahiptir. Seramik nano partiküller içeren Bioaggregate’nin antibakteriyel ve antifungal etkiye sahip olduğu, biyouyumluluğunun MTA’ya benzediği iddia edilmektedir.
