Abstract

Kök gelişimi tamamlanmamış daimi dişlerin pulpa nekrozunda kök gelişimi durmakta ve kökler kırılganlaşmaktadır. Böyle durumlarda, dişe yeniden vitalite kazandırmak ve kök gelişimini devam ettirmek amacıyla, rejeneratif endodontik tedavi uygulamalarına başvurulmaktadır. Kullanılan teknikleri; revaskülarizasyon, kök hücre tedavisi, pulpa implantı, doku iskeleleri, enjekte edilebilir scaffold uygulamaları, üç boyutlu hücre yazılımı ve gen terapisi şeklinde sıralamak mümkündür. Kan pıhtısının; hücrelerin farklılaşması ve olgunlaşmasında rol oynadığı bildirilmiştir. Rejenerasyon sağlamanın en kolay yolu, kök kanallarına erişkin kök hücrelerin enjekte edilmesidir. Pulpa implantasyonu, hücre tabakalarının büyütülmesinin kolay olması avantajına sahiptir. Doku iskeleleri; hücre canlılığının devamını sağlayacak besinleri ve büyüme faktörlerini içerir. Kök kanallarına enjekte edilen doku iskeleleri, pulpa rejenerasyonuna yardımcı olmaktadır. Üç boyutlu hücre yazılımında, doğal pulpa dokusu taklit edilebilmektedir. Mineralizasyonu arttıracak genlerin pulpaya aktarılması konusu yeterli ilgi görmemiştir. Bağışıklık sistemi zayıf, önemli sistemik veya genetik hastalığa sahip kişilerde rejeneratif endodontik tedavilerin başarılı olamayacağı iddia edilmektedir. Kullanılacak tüm tekniklerin, uzun vadeli sonuçlarının araştırılmasına ihtiyaç vardır.

In pulp necrosis of permanent teeth with incomplete root development, root development stops and the roots become brittle. In such cases, regenerative endodontic treatment is applied to restore vitality to the tooth and to maintain root development. Techniques used; revascularization, stem cell therapy, pulp implant, tissue scaffolds, injectable scaffold applications, three-dimensional cell software and gene therapy. It has been reported that the blood clot plays a role in the differentiation and maturation of cells. The easiest way to achieve egeneration is to inject adult stem cells into the root canals. Pulp implantation has the advantage that cell sheets are easy to grow. tissue scaffolds; It contains nutrients and growth factors that will ensure the continuation of cell viability. Tissue scaffolds injected into root canals help pulp regeneration. In three-dimensional cell software, natural pulp tissue can be imitated. The transfer of genes that will increase mineralization to the pulp has not received sufficient attention. It is claimed that regenerative endodontic treatments will not be successful in people with weakened immune systems and significant systemic or genetic diseases. There is a need to investigate the longterm results of all techniques to be used.
