Abstract

Ege Üniversitesi Tıp Fakültesi Parazitoloji Anabiiim Dalı Seroloji Laboratuvarına ve Akdeniz Üniversitesi Tıp Fakültesi Infeksiyon Hastalıkları Anabiiim Dalına başvuran fasciolasis varlığı kanıtlanmış 37 olgunun, kontrol grubu olarak alınan 31 sağlıklı bireyin serumu ve çapraz reaksiyon kontrol grubu olarak alınan ascariasis (6), schistosomiasis (4), toxocariasis (13), taeniasis (2), toksoplasmosis (13) ve kist hidatikli (13) hastanın serumu tarafımızdan Fasciola hepatica ES (ekskretuar sekretuar) antijenlerinden yararlanılarak hazırlanan ELISA ve DOT-ELISA, Fasciola hepatica erişkin antijenlerinden yararlanılarak hazırlanan IHA yönteminde çalışılmıştır. Uygulanan yöntemlerin duyarlılık ve özgüllüğünün hesaplanmasında fasciolasis varlığı kanıtlanmış olgular ve kontrol olguları alınmıştır. Uygulanan ELISA yönteminin duyarlılığının %100.0, DOT-ELISA yönteminin duyarlılığının %97.29, ve IHA yönteminin duyarlılığının ise %89.18 olduğu saptanmıştır. Özgüllüklerin ise ELISA yöntemi için %97, DOT-ELISA için %93.33, IHA yöntemi için %83.0 olduğu bulunmuştur. En duyarlı yöntemin ELISA (%100,0) yöntemi olduğu ve DOT-ELISA yönteminin duyarlılığının (%97.29) bu değere yakın bulunduğu, özgüllüğü en yüksek yöntemin ise ELISA (%97) yöntemi olduğu saptanmıştır. En iyi pozitif olasılık (prediktif) değere sahip yöntemin ELISA (%90,24) yöntemi olduğu, ELISA (%100,0) ve DOT-ELISA (%99,11) yöntemlerinin ise negatif olasılık (prediktif) değerlerinin yüksek bulunduğu saptanmıştır. Fasciolasis kliniği verip ancak hastalık etkeni Fasciola hepaticahm doğrudan tanısının yapılamadığı olgularda serolojik yöntemlerden en az ikisi ile tanıya gidilmesinin yerinde olacağı kanısına varılmıştır.

37 sera of confirmed patients applied to Parasitology Department of Ege University Medical Faculty and Infectious Diseases Department of Akdeniz University Medical Faculty were included to our study. Control sera groups were consisted of healthy subjects (31) and patient groups as ascariasis (6), schistosomiasis (4), toxocariasis (13), taeniasis (2), toksoplasmosis (13) and hidatidosis (13). All sera included to our study were tested by ELISA and DOT-ELISA using in-house prepared Fasciola hepatica ES (excretory-secretory) antigen and IHA test using in house prepared mature Fasciola hepatica antigen. Sensitivity and specificity of the tests were calculated with using confirmed fasciolasis patients and control sera groups. Sensitivity of the tests were found to be %100.0, %97.29 and %89.18 while specificity were found to be %97, %93.33 and %83.0 in ELISA, DOT-ELISA and IHA tests respectively. According to our results, the most sensitive test was found to be ELISA (%100,0) which was followed by DOT-ELISA (%97.29) with near values while the most specific test was found to be ELISA (%97). The highest positive predictive value-was showed by ELISA (%90,24) while the higher negative predictive values were showed by ELISA (%100,0) and DOT-ELISA (%99,1 1) tests. According to our results, we are recommending to perform at least two serological tests to clinically suspected fascioliasis cases that can not confirmed by a parasitological method.
