Abstract

Bu tez de, farklı disiplinlerin birlikte çalıştığı, karmaşık cihaz ve sistemlerin kullanıldığı radyoterapi uygulamalarında olasılıksal risk analizi yöntemi kullanılarak, riskin yüksek olduğu aşamaları değerlendirmek ve uygulama prosedürlerini iyileştirmek üzerine çalışılmıştır.Radyoterapi, hazırlanış ve uygulama açısından birçok basamaktan oluşmaktadır. Uygulanan tedavi prosedürlerinde gerçekleşmiş hata veya hatalar, geçmişten günümüze birçok radyasyon kazasına sebep olmuştur. Bu çalışmada, lineer hızlandırıcı cihazı ile radyoterapi yapan onkoloji kliniğinde olasılıksal risk analizi, hata ağacı analizi metodu ile yapılmıştır. Çalışmamızda, radyoterapi kliniğindeki iş akışı belirlenmiştir. Bu işlem, klinik uygulamanın simülasyon, tedavi planlama ve tedavi iletimi olarak üç bölüme ayrılmasıyla gerçekleştirilmiştir. Hata ağacındaki iş akışı, uluslararası kaza raporları ve klinik gözlemlerden yararlanılarak SAPHIRE yazılımı ile oluşturulmuştur. Elde edilen hata ağacının her bir dalındaki olayların olasılıkları uzman değerlendirmesi metoduyla belirlenmiştir. Sistemin zayıflıkları, geliştirilmeye ihtiyaç duyulan bölgeleri, hata dağılımı, güvenilirliği konusunda nitel ve nicel sonuçlar elde edilmiştir. Analiz sonuçları değerlendirilerek hatalı tedavi, yaralanma, hatta ölümle sonuçlanabilecek durumların riskini azaltmak amacıyla kalite yönetiminin geliştirilmesine yönelik adımlar belirlenmiştir.

In this thesis, we have worked on probabilistic risk analysis method for radiotherapy applications to evaluate the stages of high risk and to improve the implementation procedures.Radiotherapy consists of many steps in terms of preparation and application. Errors that occurred during the treatment procedures applied, radiotherapy has caused many incident from past to present. In this study, probabilistic risk analysis was performed by fault tree method (FTA) in department of radiotherapy.In this study, the workflow in the radiotherapy clinic was determined. This process was performed by dividing the clinical application into three sections as simulation (fixation and imaging), treatment planning (dose planning) and treatment delivery. The fault tree was drawn in SAPHIRE software with the help of international accident reports and clinical observations. Error probabilities were determined by the expert evaluation method and then all data entered the software. Qualitative and quantitative results were obtained on the weaknesses of the system, areas required to be improved, error distribution, and reliability. Studies were conducted on the development of quality management in order to reduce the risk of malpractice, injury or even death.
