Abstract

Disin gelisimi sürecinde amelogeninlerin ve amelinlerin, odontoblastların sonfarklılasma asamasına ve dentin olusumuna katıldıkları düsünülmektedir. Bununla birlikteodontoblastların, amelogenin proteinini aktif olarak sentezledigi ve salgıladıgıbildirilmistir.Çalısmamızdaki amaç, klinikte vital pulpa tedavilerinde sıklıkla kullanmaktaoldugumuz kalsiyum hidroksit ile periodontoloji'de kullanılan ve mine matris türevi olanEmdogain® jel ile yapılmıs vital pulpa tedavileri sonucunda olusan dentin köprülerininvarlıgını ve pulpada gözlenen degisiklikleri incelemek ve karsılastırmaktır.Çalısmada on adet kuzunun (Ovis aries) toplam altmıs adet süt alt kesici dislerikullanıldı. Disler üç gruba ayrıldı. Bir gruba herhangi bir uygulama yapılmadı ve kontrolgrubu olarak kullanıldı. Açılan pulpa yüzeylerine ise, sırasıyla kalsiyum hidroksit veEmdogain® jel uygulandı.Disler gözlem sürelerinin (1, 2, 3, 4 ve 8 hafta) sonunda rasgele ikiser gruba ayrılarakdekapite edildi ve dislerden elde edilen kesitler ısık mikroskobu altında degerlendirildi.Ayrıca ana kesitler üzerinde software programı ile kalınlık ve alan ölçümleri yapıldı.Kalsiyum hidroksit ve distile su karısımı uygulanan örneklerde, kalsiyum hidroksitiçin tipik olan histolojik görüntüler elde edilmistir. Emdogain® jel uygulanan grupta ise,gelismekte olan doku yara yüzeyinin daha altından gelismeye baslamıstır. kinci haftadasert doku olusumu baslamıstır ve degerlendirilen haftaya baglı olarak dentin köprüsüizlenmistir.Çalısmanın bulguları dogrultusunda Emdogain® jel, biyolojik olarak aktif bir maddeolarak, tamir dentini olusumuna neden olabilmektedir. Ancak olusan yeni doku, kalsiyumhidroksit ile olusan dokudan farklı sekilde gelismektedir ve olusan doku miktarlarınınkalınlık ölçümleri arasında istatistiksel olarak anlamlı bir fark bulunmamıstır (p>0.05).Alan miktarları karsılastırıldıgında ise, Emdogain® jel ile olusmus olan doku miktarıkalsiyum hidroksitten istatistiksel olarak fazla bulunmustur (p<0.05).

During odontogenesis, amelogenin and amelin are proteins that have been suggestedto participate in the final differentiation of odontoblasts and dentin formation. Also it hasbeen reported that odontoblast cells synthesis and express amelogenin proteins.The study was designed to examine whether Emdogain® jel could induce dentinalbridge formation without eliciting adverse side effects in pulp capped teeth and to comparethese treatment results, with calcium hydroxide treated dental pulps in the lamb teeth.Sixty mandibuler primary teeth of ten lambs were devided into three groups and onegroup served as controls, other teeth were treated according to their groups (Emdogain®gel, calcium hydroxide paste).Following an observation period of 1, 2, 3, 4 and 8 weeks, the lambs were randomlydevided into five groups. The extracted teeth were examined using light microscopy andsoftware measure methods.In the calcium hydroxide group, all histological sections had the classicalcharacteristic appearance of calcium hydroxide-distile water paste treated teeth. In theEmdogain® gel treated group, new tissue formation started at some distance from the apicalpart of the exposed pulp tissue. The onset of hard tissue formation could be observed after2 weeks. Due to the observation time complete hard tissue bridging could be observed.The results demonstrate that Emdogain® gel acts as a biologically active agent andinduces dentin formation. Compared with calcium hydroxide, the pattern of hard tissueformation induced by Emdogain® gel was markedly different. The thickness of new hardtissue did not show any statistically difference (p>0.05). The area of new hard tissue wasstatistically more for Emdogain® gel than the control group (p<0.05).
