Avaliação quantitativa de micronúcleos na citologia esfoliativa da mucosa bucal de pacientes dependentes químicos

Abstract

O presente trabalho avaliou quantitativamente micronúcleos (MN) na citologia esfoliativa da mucosa bucal de pacientes com dependência química. Os MN contêm material genético perdido durante a mitose em eventos clastogênicos e aneugênicos e ocorrem antes de qualquer mudança histopatológica pré-neoplásica tornar-se evidente. Foram triados 24 pacientes dependentes químicos, do CAPSAD (Centro de Apoio Psicosocial Álcool Drogas) da Prefeitura de São José dos Campos e 24 pacientes não fumantes, ou que não fumassem há 10 anos, com ingestão semanal inferior a 03 doses de bebidas alcoólicas e não usuários de drogas ilícitas, nas clínicas da FOSJCUNESP. O critério de inclusão para os grupos foi ausência de lesões bucais. Foi realizada citologia esfoliativa do bordo lingual lateral esquerdo e aplicado teste de MN com coloração de Feulgen. A análise de 600 células/paciente foi realizada por microscopia óptica e a estatística utilizou os testes Kruskal-Wallis e Mann-Whitney para freqüência de MN e células micronucleadas, e correlação de Spearman para a quantidade e anos de consumo (IC 95%, nível de significância 5%). No grupo CAPSAD a freqüência média de MN foi de 3,08 l 3,20 e a média do grupo controle foi 2,08 l 1,93; e para células micronucleadas foi 2,38 l 2,57 para o grupo CAPSAD e de 1,42 l 1,25 no grupo controle. Os resultados mostram que a freqüência de MN e células micronucleadas para o grupo CAPSAD foi numericamente superior mas a diferença entre os grupos não foi significativa estatisticamente. A quantidade diária ingerida de cigarro, álcool e drogas ilícitas mostrou maior influência na freqüência de MN que os anos de ingestão.

The aim of the present study was to evaluate quantitative micronuclei (MN) in the lining oral mucosa from chemical dependent patients. The MN originated from small extranucleus separated from the main one during cellular division. MN may appear before any clinical change is evident. Patients were divided in two groups: 24 chemical dependent patients, from CAPSAD (Centro de Apoio Psicosocial Álcool Drogas) São José dos Campos city and 24 control group patients from FOSJC-UNESP. Inclusion criterion for control group was nonsmoking for last 10 years, alcoholic beverages consumption of 03 units maximum per week, no drugs and no visible clinical alteration of the oral mucosa. Exfoliative cytology from the left side of the border of the tongue and MN test were performed. Feulgen staining was used and 600 cells were evaluated for each subject and analyzed under optical microscopies. To determinate the frequency of MN and micronucleated cells the statistical analysis used Kruskal-Wallis and Mann- Whitney tests, and Spearman correlation to determinate quantity and years of consumption (IC 95%). The incidence of MN in the CAPSAD group was 3,08 l 3,20 and in the control group was 2,08 l 1,93. The frequency of micronucleated buccal cells was 2,38 l 2,57 to CAPSAD and 1,42 l 1,25 to control group. The results showed that the micronuclei frequency and micronucleated buccal cells of CAPSAD group was increased but not statistically significant different from control group. The daily quantity consumption of cigarrete, alcohol and drugs presented effect of MN frequency higher than years of consumption.
