Efeito imediato e contínuo da informação somatossensorial adicional na sensibilidade plantar e nos parâmetros do andar de indivíduos com doença de Parkinson

Abstract

Introduction: Parkinson's disease (PD) affects directly the walking ability of patients, which leads to deficits in quality of life. Besides locomotor impairments, PD patients have deficits in processing somatosensory information. To minimize this sensory deficit, the increased plantar cutaneous stimulation has been shown to improve locomotor behavior and postural control of PD patients. However, the effects (short and long-term) of different kinds of facilitatory insoles have not been tested. Also, likely benefits after a subsequent period without the use of insoles have not been reported. Objective: to address the immediate (Study 1) and continuous effect (Study 2) of the increased plantar cutaneous stimulation on plantar sensation and gait parameters in PD patients and healthy elderly, as well as the retention of benefits after a subsequent period (follow up) without the use the insole (Study 2). Methods: Study 1 involved the participation of 19 individuals with PD and 19 neurologically healthy individuals. After clinical assessment, the locomotor behavior of the participants, with the use of facilitatory insoles (immediate effect), was assessed by kinematics and kinetics tools. Also, plantar sensation of all participants was assessed before and after the walking task protocol. Study 2 involved the participation of 19 individuals with PD and 18 healthy elderly, which were allocated in 4 groups: PDG1 - 10 PD patients who used the insole with half-spheres; PDG2 - 9 PD patients who used conventional insole; HG1 - 9 healthy elderly individuals who used the insole with half-spheres; HG2 - 9 healthy elderly individuals who used the conventional insole. Introdução: A doença de Parkinson (DP) afeta diretamente a locomoção dos pacientes, o que gera prejuízos na qualidade de vida. Além de comprometimentos locomotores, pacientes com DP apresentam déficits no processamento de informações sensoriais como, por exemplo, as provenientes do sistema somatossensorial. A fim de minimizar esse déficit sensorial, o paradigma da informação somatossensorial adicional tem apontado melhoras significativas no comportamento locomotor e no controle postural dos pacientes. Entretanto, a comparação entre diferentes informações somatossensoriais adicionais, seus efeitos imediato e contínuo e os prováveis benefícios após um período subsequente sem o uso das palmilhas ainda não foram reportados nos parâmetros do andar desta população. Objetivo: verificar o efeito imediato (Estudo 1) e contínuo (Estudo 2) do uso da informação somatossensorial adicional na sensibilidade plantar e no andar de idosos sadios e com DP, bem como a retenção dos benefícios após um período subsequente (follow up) sem o uso das palmilhas (Estudo 2). Método: O Estudo 1 contou com a participação de 19 indivíduos com DP e 19 indivíduos neurologicamente sadios. Após avaliação clínica, o comportamento locomotor dos participantes, com o uso de palmilhas facilitadoras (efeito imediato), foi analisado por meio de ferramentas cinemática e cinética. Ainda, a sensibilidade plantar de todos os participantes foi avaliada antes e após o protocolo de avaliação do andar.
