Abstract

Introdução: A hipotermia terapêutica é sabidamente um método neuroprotetor, sendo atualmente indicada em cirurgias com ausência de circulação cerebral, para pacientes que se mantêm irresponsivos após parada cardiorrespiratória e para neonatos com encefalopatia hipóxico-isquêmica moderada a grave. No entanto, sua utilização mais ampla é limitada pelos efeitos adversos relacionados à hipotermia sistêmica. Objetivos: O objetivo deste estudo é a análise da viabilidade e eficiência de um método seletivo de indução de hipotermia. Métodos: Métodos: Sete suínos foram sedados e submetidos a três métodos de indução de hipotermia (resfriamento através de capacete: grupo capacete - GC, através de catéter nasofaríngeo: grupo nasofaríngeo - GN, e a combinação desses dois métodos: grupo misto - GM), de forma subsequente, durante 60 minutos por método. Foi realizado reaquecimento ativo até alcance da temperatura basal entre os períodos de hipotermia. Os animais foram monitorizados avaliando-se as variáveis hemodinâmicas cerebrais, sistêmicas e variáveis ventilatórias, durante todo o experimento. Resultados: O GM aos 60 minutos de experimento apresentou a maior queda na temperatura cerebral quando comparado aos outros grupos (GM: 9,74°C, IQ: 1,38; GC: 6,65°C, IQ: 2,58; GN: 2,05°C, IQ: 1,38, p<0,05). O gradiente cerebral-artéria pulmonar foi maior no GM e no GC, quando comparados ao GN (GM: -6,13°C, IQ: 1,69; GC: -5,26°C, IQ: 2,72; GN: -1,17°C, IQ: 0,24, p<0,05). Não foram notadas alterações significativas de débito cardíaco, pressão arterial e frequência cardíaca entre os grupos. Conclusão: A combinação do resfriamento por cateter nasofaríngeo e capacete se mostrou eficaz, capaz de induzir hipotermia cerebral profunda com hipotermia sistêmica leve, sem alterações hemodinâmicas importantes.

Introduction: Therapeutic hypothermia is a known neuroprotective method and is currently indicated in surgeries in the absence of cerebral circulation, for patients who remain unresponsive after cardiorespiratory arrest, and for neonates with moderate to severe hypoxic-ischemic encephalopathy. However, its wider use is limited by adverse effects related to systemic hypothermia. Objectives: The aim of this study is to analyse the feasibility and efficiency of a selective hypothermia induction method. Methods: Seven pigs were sedated and submitted to three methods of induction of hypothermia (cooling through helmet: helmet group - HG, through nasopharyngeal catheter: nasopharyngeal group - NG, and the combination of these two methods: mixed group - MG), subsequently, for 60 minutes per method. Active reheating was performed until baseline temperature was reached between hypothermia periods. The animals were monitored by evaluating the cerebral hemodynamic, systemic hemodynamic and ventilatory variables throughout the experiment. Results: The MG at the 60-minute timepoint showed the greatest decrease in brain temperature when compared to the other groups (MG: 9,74°C, IQ: 1,38; HG: 6,65°C, IQ: 2,58; NG: 2,05°C, IQ: 1,38, p<0,05). The cerebral-pulmonary artery gradient was higher in GM and GC when compared to (MG: -6,13°C, IQ: 1,69; HG: -5,26°C, IQ: 2,72; NG: -1,17°C, IQ: 0,24, p<0,05). No significant changes in cardiac output, blood pressure and heart rate were observed between groups. Conclusion: The combination of nasopharyngeal catheter cooling and helmet was effective and capable of inducing deep cerebral hypothermia with mild systemic hypothermia, without important hemodynamic changes.
