Envolvimento de haptoglobina, HMGB1 e receptor CD163 de monócitos na resposta inflamatória sistêmica de gestantes portadoras de pré-eclâmpsia

Abstract

Introdução: A Pré-eclâmpsia (PE) é uma síndrome específica da gestação caracterizada por uma resposta inflamatória sistêmica, que se manifesta por ativação de monócitos do sangue periférico. O estado de ativação dessas células parece estar associado à produção de citocinas pró-inflamatórias bem como à sua interação com produtos endógenos celulares liberados durante lesão tecidual, denominados padrões moleculares associados a danos (DAMPs). High mobility group Box 1 (HMGB1) é uma citocina com potente atividade inflamatória, considerada como DAMP e secretada ativamente por monócitos. Estudos recentes mostram que HMGB1 pode interagir com a proteína de fase aguda haptoglobina, formando complexos que podem ser removidos da circulação por endocitose pelo receptor CD163 presente na superfície de monócitos/macrófagos. Objetivo: O presente trabalho teve como objetivo avaliar o envolvimento de haptoglobina, HMGB1 e receptor CD163 na resposta inflamatória sistêmica em gestantes portadoras de PE. Métodos: Monócitos obtidos de gestantes pré-eclâmpticas e de gestantes normotensas (NT) foram avaliados, por citometria de fluxo, quanto à expressão dos receptores TLR4, RAGE, CD64 e CD163 para determinar o perfil M1 ou M2 apresentado por essas células. Os níveis plasmáticos de haptoglobina, HMGB1, hemeoxigenase-1 (HO-1) e de citocinas pró e anti-inflamatórias foram determinados pela técnica de ELISA. Os resultados foram analisados por testes paramétricos ou não paramétricos com nível de significância de 5%. Resultados: Em comparação com as gestantes NT, as gestantes pré-eclâmpticas apresentaram maior expressão dos receptores de superfície de monócitos, TLR4, CD64 e RAGE e menor expressão de CD163. Os valores plasmáticos de haptoglobina, HO-1 e IL-10 apresentaram-se significativamente diminuídos nas gestantes portadoras de PE, enquanto que as concentrações de HMGB1, TNF-α, IL-1β, IL-6 e TGF-β1 foram significativamente maiores. Conclusão: A expressão aumentada dos receptores TLR4, CD64 e RAGE e a concentração endógena elevada de citocinas inflamatórias demonstram que na PE os monócitos estão polarizados para o perfil inflamatório M1. Em contrapartida, a menor expressão de CD163 e os níveis diminuídos de haptoglobina, IL-10, HO-1 em monócitos dessas gestantes sugerem deficiência nos mecanismos capazes de corrigir o intenso processo inflamatório presente na PE.

Introduction: Preeclampsia (PE) is a pregnancy-specific syndrome characterized by a systemic inflammatory response, manifested by activation of peripheral blood monocytes. The activation state of these cells seems to be associated with the production of proinflammatory cytokines as well as their interaction with cellular endogenous products released during tissue injury, called damage-associated molecular patterns (DAMPs). High mobility group Box 1 (HMGB1) is a cytokine with potent inflammatory activity considered as DAMP and actively secreted by monocytes. Recent studies show that HMGB1 can interact with the acute phase protein haptoglobin, forming complexes that can be removed from circulation by endocytosis by the CD163 receptor on the monocyte / macrophage surface. Objective: The present study aimed to evaluate the involvement of haptoglobin, HMGB1 and CD163 receptor in the systemic inflammatory response in pregnant women with PE. Methods: Monocytes obtained from preeclamptic and normotensive (NT) pregnant women were evaluated by flow cytometry for the expression of TLR4, RAGE, CD64 and CD163 receptors to determine the M1 or M2 profile presented by these cells. Plasma levels of haptoglobin, HMGB1, hemeoxygenase-1 (HO-1) and pro and anti-inflammatory cytokines were determined by the ELISA immunoassay. Results were analyzed by parametric or nonparametric tests with a significance level of 5%. Results: Compared to NT pregnant women, preeclamptic pregnant women showed higher expression of monocyte surface receptors, TLR4, CD64 and RAGE and lower expression of CD163. Haptoglobin, HO-1 and IL-10 plasma values were significantly decreased in pregnant women with PE, while HMGB1, TNF-α, IL-1β, IL-6 and TGF-β1 concentrations were significantly increased. Conclusion: The increased expression of TLR4, CD64 and RAGE receptors and the high endogenous concentration of inflammatory cytokines show that monocytes from preeclamptic women are polarized to the inflammatory profile M1. In contrast, lower CD163 expression and decreased levels of haptoglobin, IL-10, HO-1 in monocytes of these pregnant women suggest deficiency in mechanisms capable of correcting the intense inflammatory process present in PE.
