Abstract

Fundamentos: O equilíbrio homeostático do sistema imune é imprescindível para o controle e evolução do quadro clínico das neoplasias linfoides da pele e dos processos inflamatórios. Dentro desse equilíbrio imunológico, a função das células T reguladoras é controverso tanto nos casos de micose fungoide quanto nas dermatoses inflamatórias que são os principais diagnósticos diferenciais. O anti-FOXP3 é o imunomarcador mais específico para as células T reguladoras. Objetivo: Identificar e quantificar a presença de células T reguladoras através do marcador imunoistoquímico anti-FOXP3 em cortes histológicos de casos de micose fungoide em fases clínicas iniciais, e comparar com a presença ou não do mesmo marcador em casos de psoríase vulgar, dermatite eczematosa e líquen plano. Métodos: Foram estudadas 77 biópsias de 64 pacientes, sendo 30 casos de micose fungoide e 34 de dermatoses inflamatórias, consultados entre 1999 a 2014 no arquivo do serviço de Patologia da Faculdade de Medicina de Botucatu - Universidade Estadual Paulista. As biópsias foram revistas e seus resultados imunoistoquímicos comparados de acordo com as categorias entre grupo de casos e grupo controle. Resultados: A classificação do escore epidérmico para o imunomarcador FOXP3 foi significativamente menor na micose fungoide que nas dermatoses inflamatórias (p<0,001). A principal discrepância ocorreu com o líquen plano, no qual 90% foram FOXP3+ em mais de 50% dos linfócitos intraepidérmicos, enquanto que apenas 10% dos casos de micose fungoide alcançaram esses níveis de positividade. Conclusões: Embora a maioria dos diagnósticos das lesões iniciais de micose fungoide dependam principalmente da correlação clínica-histopatológica, a diferença estatisticamente significante da proporção de FOXP3+ intraepidérmico comparando-se com as dermatoses inflamatórias permite a inclusão desse imunomarcador no painel imunoistoquímico para o...

Background: The homeostatic balance of the immune system is essential for clinical evolution and control of cutaneous lymphoid neoplasias and inflammatory diseases. Regarding immunological balance the role of regulatory T cells remain unclear either in mycosis fungoides or their clinical differential diagnosis like inflammatory dermatosis. Anti- FOXP3 antibody is the most specific marker of regulatory T cells. Objective: To determine and quantify regulatory T cells using anti-FOXP3 antibody in histopathological samples of early lesions of mycosis fungoides and compare it with psoriasis vulgaris, eczematous dermatitis and lichen planus by a scoring system for FOXP3 expression. Methods: Data from 77 biopsies from 64 patients, 30 cases corresponding to mycosis fungoides and 34 to inflammatory dermatosis, observed between 1999 and 2014 at the Department of Patholgy at Botucatu Medical School - Sao Paulo State University. Biopsies were reviewed and the FOXP3 labeled cells results compared according to cases or control group. Results: Intraepidermal FOXP3 positivity was significantly lower in mycosis fungoides than inflammatory dermatosis (p<0,001). Especially significant was that 90% of lichen planus cases resulted in 50% or more of epidermal lymphocytes scored FOXP3+ whereas only 10% of mycosis fungoides group have reached this level. Conclusions: Despite the majority of early lesions of mycosis fungoides diagnosis depend mainly on clinical and histopathological correlation, the statistical significance of intraepidermal FOXP3 positivity comparing with inflammatory dermatosis allow us to include this antibody to help defining the diagnosis or giving support to suspect cases morfologically dubious of mycosis fungoides
